In un contesto dove tantissimi studi sono ongoing, molti risultati stanno emergendo e tantissimi quesiti restano ancora in sospeso … poteva essere utile avere un dato metanalitico. In realtà, purtroppo, questa metanalisi non ci fornisce grosse risposte, ma è comunque un riassunto dei principali trials, in modo trasversale, con diverse molecole, differenti setting e istologie.
To be in pain (or not): a computer enables outpatients to inform their physician
In the outpatient oncology clinic, pain management is often inadequate. Incorporating a systematic pain management program into visits is likely to improve this. We implemented an integrated program, including a structured pain assessment, pain treatment protocol and patient education module. L’attenzione alla rilevazione e al trattamento del dolore sono ormai entrati diffusamente nella pratica oncologica: qualche decennio fa, numerosi report sottolineavano l’elevato rischio di sottotrattamento del dolore, che oggi è sicuramente migliorato. L’esperienza olandese, recentemente pubblicata su Annals of Oncology, ci ricorda che un aiuto nella rilevazione ottimale del dolore può venire anche dagli strumenti elettronici: i pazienti avevano la possibilità di registrare l’intensità del loro dolore direttamente su un computer “touch-screen” e la loro risposta veniva automaticamente registrata nella cartella clinica. Il risultato interessante è che, una volta integrato tale sistema nella pratica clinica, la percentuale di pazienti con dolore moderato-severo si è ridotta, nell’arco di 6 mesi, dal 12,5% all’8,5%. Naturalmente, non trattandosi di un confronto diretto tra tale sistema “informatico” di rilevazione del dolore e la “classica” visita con la semplice domanda da parte del medico, non sappiamo “quantificare” la quota di miglioramento del dolore realmente attribuibile alla procedura elettronica. Peraltro, in generale, tutte le iniziative che mirano a ottimizzare il report dei sintomi e delle tossicità soggettive da parte dei pazienti sono sicuramente da incoraggiare.

In this issue of JAMA, the US Preventive Services Task Force (USPSTF) provides recommendations for screening for skin cancer. L’US Preventive Services Task Force (USPSTF) conclude questo articolo enunciando la non utilità dell’esame della pelle come metodo di prevenzione. Nel paper si fa riferimento ad un unico studio (SCREEN) con importanti limiti metodologici, che non può essere (e non è) sufficiente per un’indicazione clinica. Ad oggi, secondo una casistica statunitense raccolta nel 2005, l’esame della pelle viene praticato sui pazienti dall’81% dei dermatologi, 60% medici di base e 56% degli internisti e consiste fondamentalmente nell’esame di lesioni cutanee asimmetriche, con margini irregolari, colore non-uniforme, diametro superiore ai 6 mm e con cambiamento (dimensionale e/o morfologico) nel tempo. Dall’articolo questa la tabella estrapolata: Risk Assessment Skin cancer occurs more commonly in men than in women and among persons with a fair complexion, persons who use indoor tanning beds, and persons with a history of sunburns or previous skin cancer. Specific risk factors for melanoma include having a dysplastic nevus (atypical mole), multiple (≥ 100) nevi, and a family history of melanoma. Risk of melanoma also increases with age. Screening tests The clinical visual skin examination assesses skin lesions using the “ABCDE rule”, which involves looking for the following characteristics: asymmetry, border irregularity, nonuniform color, diameter > 6 mm, and evolving over time. Treatment and Interventions Treatment of screen-detected melanoma generally involves excision, with or without lymph node management, depending on the stage at diagnosis. There are a variety of treatments available for squamous and basal cell carcinoma, including surgical excision, Mohs micrographic surgery, radiation therapy, curettage and electrodessication, and cryosurgery. Balance of Benefits and Harms The HSPSTF concludes that the current evidence is insufficient and that the balance of benefit and harms of visual skin examination by a clinician to screen for skin cancer in asymptomatic adults cannot be determined. Other Relevant USPSTF Recommendations The USPSTF recommends that children, adolescents, and young adults aged 10 to 24 years who have fair skin be counseled about minimizing their exposure to ultraviolet radiation to reduce their risk of developing skin cancer. Vemurafenib in patients with BRAFV600E-positive metastatic or unresectable papillary thyroid cancer refractory to radioactive iodine: a non-randomised, multicentre, open-label, phase 2 trial
About half of patients with papillary thyroid cancer have tumours with activating BRAFV600E mutations. Vemurafenib, an oncogenic BRAF kinase inhibitor approved for BRAF-positive melanoma, showed clinical benefit in three patients with BRAFV600E-positive papillary thyroid cancer in a phase 1 trial. Nonostante la maggior parte dei tumori tiroidei siano guaribili, il 30-40% delle neoplasie divengono refrattarie al trattamento con iodio radioattivo (I-131). Nell’ultimo quinquennio due nuove molecole hanno rivoluzionato il panorama del trattamento della patologia radioresistente: sorafenib (Brose MS et al, Lancet 2014) e lenvatinib (Schlumberger M et al, N Engl J Med 2015); nonostante i progressi terapeutici la neoplasia sviluppa resistenza al trattamento con i TKI ed alcuni pazienti manifestano intolleranza al trattamento. Lo studio di fase 2 recentemente pubblicato su Lancet Oncology sfrutta una importante caratteristica biologica della malattia: la mutazione di BRAFV600E è presente in quasi il 50% dei tumori papilliferi, dove correla con un andamento più aggressivo e con una aumentata radioresistenza. Nel trial sono state arruolate due coorti di pazienti con PS ECOG 0-1, trattate con vemurafenib alla dose di 960 mg bid. L’impressione è che la prima coorte (26 pazienti naïve da terapia sistemica con inibitori multitarget) abbia avuto un outcome in termini di tasso di risposte, durata mediana del controllo di malattia, PFS mediana e OS mediana migliore della seconda (che includeva 25 soggeti pretrattati con inibitori di VEGFR). In entrambe le coorti, due terzi dei pazienti riportavano eventi avversi severi (insorgenza di tumori squamosi della cute, linfopenia, fatigue e dispnea) e un quarto dei pazienti interrompevano il trattamento a causa di effetti collaterali. Sebbene lo studio abbia un numero limitato di pazienti inclusi, l’attività del vemurafenib in pazienti con neoplasia tiroidea resistente al trattamento con radioiodio apre una nuova opzione terapeutica. Rimangono da stabilire se sia necessario lo screening molecolare per BRAF e quale sia la migliore sequenza terapeutica nei casi con mutazione V600E.
