Vengono riportati i dati della parte 1 dello studio CheckMate 227, di fase III, sull’utilizzo dell’associazione di nivolumab ed ipilimumab nei pazienti con NSCLC avanzato e “high” mutational burden (≥ 10 mutazioni per megabase). In questa sottopopolazione di pazienti, la combinazione di farmaci immunoterapici (nivolumab+ipilimumab) ha dimostrato un miglioramento della sopravvivenza libera da progressione rispetto alla chemioterapia contenente platino, indipendentemente dal livello di espressione del PD-L1 e dall’istologia (squamosa e non-squamosa); con un tasso di sopravvivenza libera da progressione ad un anno del 42,6% con l’associazione vs 13,2% con la chemioterapia ed una sopravvivenza libera da progressione mediana di 7,2 mesi nel gruppo trattato con nivolumab+ipilimumab vs 5,5 mesi nel gruppo trattato con chemioterapia (hazard ratio [HR] = 0,58; p < 0,001). Al contrario, pazienti con “low” mutational burden trattati con nivolumab+ipilimumab presentano una sopravvivenza libera da progressione mediana di 3,2 mesi vs 5,5 mesi per la chemioterapia. Lo studio conferma pertanto il ruolo del tumor mutational burden come biomarcatore per la selezione dei pazienti da trattare con associazione di immuno-checkpoint inibitori. Eventi avversi di grado 3 e 4 sono stati descritti nel 31,2% dei pazienti trattati con la combinazione e nel 36,1% dei pazienti trattati con chemioterapia. L’interruzione del trattamento per tossicità è stata registrata nel 17,4% vs 8,9% dei pazienti trattati con la combinazione di immunoterapici e chemioterapia rispettivamente. Gli eventi avversi hanno causato la morte in 7 pazienti (1,2%) del gruppo trattato con la combinazione di immunoterapici e in 6 pazienti (1,2%) in trattamento con chemioterapia. Questi dati dimostrano che la combinazione di nivolumab+ipilimumab migliora la sopravvivenza libera da progressione dei pazienti affetti da NSCLC avanzato con “high” tumor mutational burden (indipendentemente dal livello di espressione del PD-L1) e confermano di quest’ultimo il ruolo di biomarcatore per la selezione dei pazienti. L’associazione di due farmaci ad azione immunitaria è sicuramente di grande interesse e, fino ad ora, i dati in diversi setting per quanto riguarda il trattamento del tumore polmonare non sono stati cosi promettenti. Questo è sicuramente un segnale di efficacia e può generare ipotesi di ricerca interessanti. Resta sicuramente il problema dell’implementazione del test in una routine diagnostica già di per sé molto “affollata” e con non pochi cambiamenti appena occorsi e tuttora in essere.

Si tratta di uno studio osservazionale multicentrico su 188 pazienti sottoposti a trattamento chemioterapico per NSCLC Stadio IV nelle quali veniva effettuata prima del I ciclo di trattamento (T1) fino a prima del IV (T3) la rilevazione dei sintomi su scala numerica, in particolare ansia, umore, stanchezza, appetito, nausea, vomito, dolore, sonnolenza e condizioni generali. La valutazione veniva effettuata dal medico, dall’infermiere e dal paziente con una successiva stima di concordanza (Medico-Paziente, Infermiere-Paziente, Medico-Infermiere) tramite coefficiente kappa di Cohen. Questo lavoro, italiano, riprende un’importante esperienza navigata del gruppo di Ethan Basch, pubblicata nel 2006 sul Lancet Oncology (leggi) e successivamente ripetuta in Italia (leggi), volta a sottolineare l’importanza dei PROs nel symptom assessment. In questo caso l’attenzione viene posta dagli autori sulla Nausea e Vomito indotti dalla chemioterapia (CINV) evidenziando come nello studio l’incidenza dei pazienti che hanno riportato CINV variava dal 40% al T0 al 71% al T3. Gli autori dimostrano come la concordanza nel symptom assessment sia stata soddisfacente (moderate) sia per i medici che per gli infermieri, con un leggero aumento della concordanza stessa nel tempo per alcuni sintomi (substantial). L’ansia prima del trattamento chemioterapico nelle sue manifestazioni lievi, moderate e gravi, così come l’ansia lieve, moderata e grave, sperimentata dopo l’inizio della chemioterapia, ha esposto i pazienti a un rischio più elevato di CINV sia anticipatoria sia post-trattamento. Lo studio dimostra inoltre come, anche con uno Staff consapevole e sensibile sullo stato e le percezioni dei pazienti, la CINV rappresenti ancora un problema tra i pazienti in trattamento chemioterapico e all’importante ruolo della sfera emotiva nello sviluppo della stessa.

Questa è la prima metanalisi/revisione sistematica che valuta l’effetto prognostico del ki67 nei pazienti affetti da tumore della vescica non muscolo infiltrante trattati con immuno-terapia intravescicale (BCG). Sono stati analizzati 11 studi che includevano 1.320 pazienti: tale valutazione conferma che non esiste una correlazione tra espressione in immunoistochimica del ki67 e sopravvivenza libera da recidiva, mentre si dimostra essere un fattore prognostico per la sopravvivenza libera da progressione in maniera statisticamente significativa (HR 2,922; IC 95%: 2,002 – 4,266). Nonostante ciò molti fattori determinano una importante eterogeneità nelle analisi di sottogruppo tra cui la provenienza regionale e la razza dei pazienti, le modalità operative nell’effettuazione della resezione transuretrale (TUR) e nelle instillazioni vescicali del BCG, nonché gli stessi studi selezionati ed inseriti in tale analisi. I risultati di questo studio possono però aiutare a ricordare ai clinici che per i pazienti con alta espressione di ki67 potrebbe essere necessario sviluppare piani di follow-up più personalizzati, ossia più brevi, nonché modificare prontamente la strategia di trattamento.

In questa pubblicazione sul Journal of Clinical Oncology gli autori evidenziano come i farmaci cosi detti “rivoluzionari” non necessariamente sono caratterizzati da nuovo meccanismo d’azione. L’analisi viene condotta su 58 molecole approvate da FDA nel periodo fra Gennaio 2012 e Dicembre 2017. In questa finestra temporale le terapie designate come “Breakthrough Therapy” sono state 25 (43%). Il tempo mediano alla prima approvazione FDA è stato pari a 5,2 anni per questa classe di farmaci verso 7,1 per quelli “non rivoluzionari” (differenza pari a 1,9 anni; p = 0,01). Per “breakthrough-designated” vs “non-breakthrough-designated”, il vantaggio in median progression-free survival (mPFS) è stato di 8,6 vs 4,0 mesi (p = 0,11), hazard ratios per PFS 0,43 vs 0,51 (p = 0,28), e il response rate per i tumori solidi era 37% vs 39% (p = 0,74). I farmaci “breakthrough-designated” e “non-breakthrough-designated” hanno pressochè uguale probabilità di essere caratterizzati da nuovo meccanismo d’azione (36% vs 39%; p = 1,00). I tassi di mortalità (6% vs 4%; p = 0,99) e le percentuali di eventi avversi (38% vs 36%; p = 0,93) sono pressochè sovrapponibili nelle due categorie. In conclusione, la differenza maggiore sta sicuramente nei tempi di approvazione, apparentemente legata ad un’etichetta apposta su un farmaco piuttosto che su un altro, più che a reali differenze di efficacia, tollerabilità e/o meccanismo d’azione.
