Lo studio suggerisce la potenziale utilità di dasatinib, un potente inibitore di BCR-ABL, KIT, e delle chinasi della famiglia SRC, somministrato alla dose di 70 mg bid, nel controllo di malattia per pazienti con GIST non resecabile e resistente a imatinib. Gli autori riportano, inoltre, un tasso di risposta del 25% documentata localmente utilizzando i criteri di Choi. Lo studio, tuttavia, ha arruolato solo 50 pazienti e non prevedeva un braccio di controllo. I dati riportati sono in linea con i risultati del fase II condotto in pazienti TKI-naïve dallo Swiss Group for Clinical Cancer Research e recentemente pubblicati su Cancer; rimane da stabilire in quale subset molecolare di pazienti il trattamento possa essere di maggior beneficio. La sequenza di trattamento disponibile nella pratica clinica per pazienti con malattia avanzata non viene tuttavia modificata: imatinib – con dose di 400/800 mg dipendentemente dal make-up molecolare della malattia-, sunitinib, regorafenib (vedi linee guida AIOM su sarcomi dei tessuti molli e GIST, versione 2017). Il dasatinib, inoltre, non è registrato in Italia con questa indicazione.

Questo studio multicentrico ha valutato, in pazienti affetti da carcinoma della prostata metastatico resistente alla castrazione, il vantaggio in termini di sopravvivenza cancro-specifica del trattamento con farmaci sperimentali nell’ambito di trial clinici rispetto al trattamento con terapie standard. Quest’analisi retrospettiva ha incluso 299 pazienti con nuova diagnosi di carcinoma prostatico tra settembre 2009 e marzo 2017, 65 dei quali sono stati arruolati in trial clinici che prevedevano un trattamento sistemico con ormonoterapia, chemioterapia o farmaci radiometabolici per CRPC. Ad un follow up mediano di 16 mesi il tasso di sopravvivenza cancro-specifica a 2 anni è risultato superiore nei pazienti arruolati in trial clinici (61,3 vs 42,4%; p = 0,003) rispetto ai pazienti che hanno ricevuto terapie standard. Lo studio suggerisce come l’arruolamento in studi clinici, laddove possibile, dovrebbe essere preso in considerazione in pazienti affetti da carcinoma prostatico resistente alla castrazione. I principali limiti dello studio sono il numero esiguo di pazienti considerati e il bias di selezione dei pazienti come conseguenza della natura retrospettiva dell’analisi; i pazienti arruolati in trial clinici presentavano infatti, come prevedibile, caratteristiche basali più favorevoli (es: ECOG PS, PSA e albumina basale).
Mechanisms and clinical activity of an EGFR and HER2 exon 20-selective kinase inhibitor in non-small cell lung cancer

Il gruppo di John V. Heymach presso il Dipartimento di Oncologia Toracica dell’MD Anderson Cancer Center riporta i risultati di questo studio su Nature Medicine. Poziotinib viene descritto come inibitore attivo di mutazioni a carico dell’esone 20 per EGFR e HER2 in casi di carcinoma polmonare non a piccole cellule in stadio avanzato. Indubbiamente questo colma una necessità nella pratica clinica, che vede le mutazioni attivanti (e comuni) di EGFR essere responsive agli inibitori TK, ma le mutazioni sull’esone 20 come intrinsecamente resistenti. Lo studio si è svolto in alcuni centri statunitensi con una parte di ricerca traslazionale (modelli in vitro e in vivo) volta a valutare la capacità di poziotinib di bypassare grazie alla sua conformazione (e dimensione) il problema di legame con la tasca recettoriale: i primi 11 pazienti con mutazioni di EGFR a carico dell’esone 20 hanno ricevuto poziotinib conseguendo una risposta pari al 64%. I dati sono sicuramente preliminari, ma aggiungono un altro tassello al complesso disegno della medicina di precisione applicata all’oncologia toracica.

Lo studio multicentrico analizza 1.668 donne con carcinoma dell’endometrio in stadio I-II e dimostra che nell’11% di loro sono presenti cellule neoplastiche nel liquido di lavaggio peritoneale. Nella precedente classificazione della FIGO la presenza di washing peritoneale (WP) positivo era riconosciuta come fattore di rischio e condizionava un upstaging della malattia con conseguenza sulla scelta dell’adiuvante. L’attuale classificazione ha rimosso il WP dalla stadiazione non considerandolo più un fattore prognostico sfavorevole ma il mondo scientifico ha sempre mantenuto qualche perplessità su questo aspetto. Questo studio avalla le perplessità, perché la presenza di WP positivo per cellule neoplastiche nel carcinoma iniziale a basso rischio rappresenta un fattore di rischio per la ripresa di malattia a distanza e per la sopravvivenza e credo vada discusso con la paziente e considerato nella scelta dell’adiuvante.

In questa metanalisi pubblicata su Lancet Oncology viene sottolineato l’aumentato rischio di tumori del tratto gastrointestinale (GI) nei pazienti affetti da fibrosi cistica (FC) rispetto al dato nella popolazione generale. È ovvio che questo dato si è reso manifesto con l’aumentata aspettativa di vita dei pazienti affetti da fibrosi cistica. La review analizza 99.925 pazienti da una revisione della letteratura condotta nell’agosto 2017 comprendente 6 studi, il cui obiettivo era proprio l’analisi del rischio di neoplasie GI in pazienti con FC. Il rischio si è rivelato essere superiore nei pazienti con FC (pooled standardized incidence ratio [SIR] = 8,13; p < 0,0001) e, più nel dettaglio, per i pazienti con pregresso trapianto polmonare il rischio è risultato superiore rispetto agli altri pazienti con FC (pooled SIR = 21,13; p < 0,0001, vs pooled SIR = 4,18; p < 0,0001). Andando più nel dettaglio, questi i rischi per specifica patologia GI: intestino tenue (pooled SIR = 18,94; p < 0,0001), colon (pooled SIR = 10,91; p < 0,0001), biliare (pooled SIR = 17,87; p < 0,0001), pancreatico (pooled SIR = 6,18; p = 0,022). I casi di fibrosi cistica dell’adulto sono aumentati negli ultimi anni, creando tra l’altro anche la problematica dei centri di riferimento e pochi sono tuttora i centri in Italia. La FC, già per sue caratteristiche, comporta la continua integrazione e collaborazione di diverse figure specialistiche, cui evidentemente andrà aggiunto l’oncologo medico, cosi come andranno indubbiamente valutate specifiche campagne di informazione e prevenzione per questi pazienti.
