TAILORx: Many Women With Early-Stage Breast Cancer Can Avoid Chemotherapy

Al congresso ASCO 2018 sono stati presentati, e simultaneamente pubblicati su NEJM, i risultati finali dello studio TAILORx, che ha coinvolto oltre 10.000 donne con carcinoma mammario operato HR+/HER2-, T1-3, N0. I risultati dello studio dimostrano che per le donne con un Recurrence Score < 25, che rappresentano circa il 70%, può essere risparmiata la chemioterapia adiuvante in aggiunta alla terapia ormonale. Lo studio, che ha avuto un’importante risonanza, è rassicurante e conferma in qualche modo la convinzione sempre più crescente tra gli oncologi che la maggior parte delle donne con tumore mammario HR+/HER2- N0 non necessitano di chemioterapia adiuvante. Per quanto concerne l’utilizzo del test genomico per l’identificazione di queste pazienti cui risparmiare il trattamento chemioterapico, il quesito più importante che rimane aperto è a quali pazienti proporlo. Infatti, almeno in Italia, l’uso della chemioterapia in questo setting è già ristretto a quelle pazienti con caratteristiche clinicopatologiche più sfavorevoli. Ad oggi i test genomici non sono approvati in Italia e non sarebbe possibile pensare di offrire il test a tutte le donne con una diagnosi di tumore mammario HR+/HER2- N0 in quanto questo atteggiamento ne limiterebbe l’impatto in termini di cambio della decisione terapeutica (essendo l’indicazione alla sola terapia ormonale già frequente), gravando in modo importante sui costi. È quindi importante, anche sulla base delle diverse esperienze Italiane con l’utilizzo di questi test, identificare la popolazione ottimale.

EMA restricts use of Pembrolizumab and Atezolizumab in bladder cancer
June 1, 2018 – Early data from two clinical trials1 show reduced survival with Pembrolizumab and Atezolizumab when used as first-line treatments for urothelial cancer (cancer of the bladder and urinary tract) in patients with low levels of a protein called PD-L1. The data indicate that Pembrolizumab and Atezolizumab may not work as well as chemotherapy medicines in this group of patients. La Sezione Regionale AIOM Liguria ha istituito il Premio ‘Giovanni Gardin’ 2018, riservato a Giovani Oncologi di età non superiore ai 40 anni (nell’anno solare in corso), quale riconoscimento per i migliori progetti su argomenti a cui il dottor Giovanni Gardin ha dedicato parte della propria attività clinica.
I premi saranno due, uno dedicato a un progetto su ‘I Tumori Cerebrali in Liguria’ e l’altro dedicato a un progetto sulla ‘Identificazione di Fattori Prognostici/Predittivi nei Tumori Solidi in Setting Adiuvante o Avanzato’. A ciascuno dei due migliori progetti di studio presentati sarà assegnato un premio pari a 4.000 Euro (al lordo della ritenuta fiscale). I premi saranno assegnati in occasione del Congresso Regionale AIOM Liguria.
La deadline per la domanda di partecipazione è il 31 luglio 2018.
