Al congresso ASCO 2018 sono stati presentati, e contemporaneamente pubblicati sul NEJM, i risultati dello studio CARMENA, studio randomizzato di fase 3 volto a definire il ruolo della nefrectomia citoriduttiva in pazienti affetti da carcinoma renale a cellule chiare metastatico trattati con sunitinib. 450 pazienti affetti da carcinoma a cellule chiare renali metastatico a rischio intermedio o poor sono stati randomizzati 1:1 ad intervento di nefrectomia seguito da terapia con sunitinib (50 mg al giorno con schedula 4:6) versus la sola terapia con sunitinib. Il trattamento con il solo sunitinib è risultato non inferiore rispetto alla nefrectomia seguita da sunitinib, con una sopravvivenza mediana di 18.4 vs 13.9 mesi nei due bracci. Non sono state dimostrate differenze significative nemmeno in termini di sopravvivenza libera da progressione e tasso di risposte obiettive. Si tratta di uno studio practice-changing, i cui risultati contrastano con quelli dei precedenti studi retrospettivi, che avevano dimostrato un vantaggio in OS a favore della nefrectomia prima della terapia sistemica con IFN- versus la sola terapia con IFN-. Il poter evitare un atto chirurgico non necessario elimina per il paziente inutili rischi di mortalità, morbidità e ritardo nell’inizio di una terapia sistemica efficace. Un limite di questo studio è sicuramente legato al modello prognostico utilizzato che ormai risulta sorpassato dal sistema prognostico di Heng. Inoltre l’esclusione, a discrezione dell’investigatore, dei pazienti con basso tumor burden potrebbe essere considerato come un potenziale bias, e ciò potrebbe avere contribuito all’elevata percentuale di pazienti poor prognosis e alla breve OS.

L’articolo conferma quanto già riportato da almeno tre altre segnalazioni di letteratura negli ultimi due anni: le pazienti con carcinoma ovarico devono essere trattate in centri di riferimento da team multidisciplinari, perchè questo si traduca in un miglioramento della prognosi in termini di anni di vita. Questo era già noto per la parte legata alla terapia medica, per la possibilità di offrire a queste pazienti protocolli sperimentali e la possibilità di cura che altrove sarebbero precluse, ma diventa ancora più importante per la terapia chirurgica che richiede una expertice importante che si traduce in una percentuale di citoriduzione ottimale più alta e in anni (30 mesi mediamente) di vita guadagnata.

Vi è ormai evidenza che l’immunoterapia si attiva nel carcinoma gastrico sebbene in una percentuale di pazienti compresa tra il 10 e il 15%. Una parte di essi è sicuramente caratterizzata dai pazienti con tumori MSI-high che comunque non superano il 4-5% del totale. La restante parte dei pazienti resta ancora non accuratamente selezionabile sulla base dei dati disponibili (es. PD-L1 status). Negli all comers non selezionati, pembrolizumab non ha migliorato la sopravvivenza mediana rispetto al taxolo, sebbene la tollerabilità del trattamento sia migliore. Nonostante le mediane di sopravvivenza si equivalgano, se si osservano le code delle curve è palese come ci sia il solito 10% di pazienti che beneficia a lungo termine e in maniera significativamente migliore della chemioterapia, e l’identificazione di tali pazienti soprattutto nell’ambito dei tumori MSS consentirebbe di migliorare notevolemente il profilo costo-beneficio dell’immunoterapia. Sicuramente la selezione clinica (pazienti con PS 0) sembra migliorare la predizione dell’efficacia, stante anche l’aggressività della patologia di base. Forse l’associazione con la chemioterapia potrebbe avere un ruolo. Di fatto lo sviluppo di biomarker potrebbe ricevere davvero impulso in questa patologia addirittura prima di altre, forse se le companies si vedranno negare l’indicazione/rimborsabilità e vorranno effettuare studi biomarkers-driven che, pur restringendo il target di mercato, possano effettivamente portare a una miglior sostenibilità ed efficacia del trattamento.

La Sezione Regionale AIOM Liguria ha istituito il Premio ‘Giovanni Gardin’ 2018, riservato a Giovani Oncologi di età non superiore ai 40 anni (nell’anno solare in corso), quale riconoscimento per i migliori progetti su argomenti a cui il dottor Giovanni Gardin ha dedicato parte della propria attività clinica.
I premi saranno due, uno dedicato a un progetto su ‘I Tumori Cerebrali in Liguria’ e l’altro dedicato a un progetto sulla ‘Identificazione di Fattori Prognostici/Predittivi nei Tumori Solidi in Setting Adiuvante o Avanzato’. A ciascuno dei due migliori progetti di studio presentati sarà assegnato un premio pari a 4.000 Euro (al lordo della ritenuta fiscale). I premi saranno assegnati in occasione del Congresso Regionale AIOM Liguria.
La deadline per la domanda di partecipazione è il 31 luglio 2018.
