Questo aggiornamento delle linee guida americane sul trattamento del carcinoma pancreatico metastatico, a due anni dalle precedenti, ha come obiettivo principale quello di incorporare i nuovi dati circa le opzioni disponibili in seconda linea, considerando i dati più recenti e i cambiamenti avvenuti negli ultimi anni anche nel trattamento della prima linea metastatica. La novità più importante delle nuove raccomandazioni riguarda l’esecuzione routinaria del test per la ricerca di deficit nelle proteine del mismatch repair o della instabilità dei microsatelliti (rilevabili nello 0,8% circa di tutti i tumori del pancreas) per selezionare i pazienti candidabili a immunoterapia con pembrolizumab, farmaco approvato da FDA per questa categoria di pazienti sulla base dei dati dello studio di Le et al pubblicato nel 2017 (in particolare risposta radiologica completa in 2/8 pazienti affetti da carcinoma del pancreas con un tasso di controllo di malattia del 75%) (leggi abstract). Per i pazienti non portatori di deficit nelle proteine di riparazione del DNA, che rappresentano di lunga la maggior parte, rimangono valide le opzioni chemioterapiche a base di 5-fluorouracile da associare a oxaliplatino o, qualora disponibile, a irinotecan nanoliposomiale.
Response rates to second-line platinum-based therapy in ovarian cancer patients challenge the clinical definition of platinum resistance
The aim of this study was to compare response rates and survival in women with “platinum resistant” epithelial ovarian cancer (EOC) who received further platinum-based or non platinum chemotherapy for treatment at first relapse. Patients with high-grade EOC (including fallopian tube and peritoneal cancer) of all histologies recruited to the Australian Ovarian Cancer Study (AOCS) and treated with platinum-based primary chemotherapy were included. Lo studio ha analizzato la storia clinica, la risposta ai trattamenti e la sopravvivenza globale di 341 pazienti, definite platino resistenti (cioè con ricaduta di malattia a < 6 mesi dalla fine del trattamento a base di platino), trattate in seconda linea con differenti regimi di chemioterapia. Mentre nelle pazienti con recidiva tra 0 e 3 mesi dalla fine del platino non vi era alcuna differenza di risposta se si utilizzavano trattamento a base di platino o agenti non platino, nelle pazienti che ricadevano tra 3 e 6 mesi il trattamento a base di platino si traduceva in una maggiore risposta e migliore sopravvivenza rispetto al non platino. Questo studio, che necessita di conferma, tenderebbe a suggerire che la soglia che definisce la platino resistenza andrebbe anticipata a 3 mesi e non a 6 come fino ad adesso è stato fatto. In realtà vale la pena ricordare che le soglie sono comunque arbitrarie e molto legate a come si fa la sorveglianza nel follow up. Oltretutto, il tempo alla recidiva è fortemente condizionato dal tipo di chirurgia (chirurgia primaria vs neoadiuvante) e chemioterapia (con o senza bevacizumab) che la paziente ha ricevuto in prima linea. Pertanto è il giudizio del clinico, come ulteriormente sottolineato nella V conferenza mondiale sul tumore dell’ovaio, che deve guidare la scelta del trattamento con o senza platino al momento della recidiva.

Quest’analisi post-hoc dello studio TRIBE costituisce sicuramente una buona ipotesi su cui lavorare in studi futuri. È infatti noto l’effetto prognostico sfavorevole della localizzazione destra del tumore primitivo nel carcinoma del colon-retto metastatico, ma anche il suo effetto predittivo negativo per quanto riguarda l’efficacia dei farmaci anti-EGFR, ed infine possibilmente predittivo positivo per quanto riguarda l’intensificazione della chemioterapia con il regime FOLFOXIRI. In questa analisi, l’effetto prognostico della localizzazione destra sembra perdersi quando viene considerato “al netto” delle mutazioni di RAS e BRAF che sono notoriamente iper-rappresentate nel colon di destra. Questo lavoro, seppur con i limiti dei piccoli numeri di pazienti in ciascun sottogruppo, ha quindi il pregio di effettuare un’analisi che associa le caratteristiche molecolari della malattia al “surrogato clinico” costituito dalla sede del tumore, in una popolazione inizialmente non selezionata dal punto di vista molecolare.

Scopo della survey condotta in Inghilterra su oltre 1.300 soggetti, e pubblicata su European Journal of Cancer, era quello di valutare le conoscenze delle persone relativamente ad alcuni fattori di rischio accertati di tumore, nonché relativamente ad alcune cause “mitiche”, vale a dire notizie prive di una reale dimostrazione scientifica. Strumento usato per la misura della conoscenza dei fattori di rischio noti di tumore era la Cancer Awareness Measure (CAM), vale a dire uno strumento validato. Al contrario, non disponendo di uno strumento validato per la misura della conoscenza delle cause non dimostrate di cancro, gli autori hanno messo a punto uno strumento chiamato CAM-MYthical Causes Scale (CAM-MYCS). Una proporzione variabile di rispondenti non ha dimostrato conoscenza di alcuni fattori di rischio accertati di tumore (come l’alcool o il papillomavirus, o il consumo di carni rosse, o la scarsa attività fisica, o lo scarso consumo di frutta e verdura) e, al tempo stesso, una certa proporzione di rispondenti ha definito come accertati alcuni fattori la cui associazione con il cancro è assolutamente non provata (come l’impiego di forni a microonde o bere acqua da bottiglie di plastica). Anche se per certi aspetti è comprensibile che le persone siano turbate da notizie di possibili associazioni tra alcuni fattori di rischio e il cancro, sarebbe molto meglio che tale attenzione venisse concentrata sulle notizie serie e scientificamente attendibili. Il carcinoma dei dotti collettori del Bellini costituisce una patologia rara ed orfana, dalla prognosi estremamente sfavorevole dopo fallimento chirurgico. Esperienze limitate includenti chemioterapia con differenti regimi e targeted therapies hanno dato risultati contrastanti e soprattutto non conclusivi in merito al miglior trattamento terapeutico nella malattia avanzata. Gli autori presentano i risultati di uno studio prospettico teso a valutare l’efficacia della combinazione tra sorafenib, cisplatino e gemcitabina in tale patologia sfuggita ad un approccio locale. Incoraggianti i dati di efficacia riportati seppur accompagnati a non trascurabili effetti collaterali. Tale popolazione andrebbe preferibilmente candidata a trial clinici.

I premi saranno due, uno dedicato a un progetto su ‘I Tumori Cerebrali in Liguria’ e l’altro dedicato a un progetto sulla ‘Identificazione di Fattori Prognostici/Predittivi nei Tumori Solidi in Setting Adiuvante o Avanzato’. A ciascuno dei due migliori progetti di studio presentati sarà assegnato un premio pari a 4.000 Euro (al lordo della ritenuta fiscale). I premi saranno assegnati in occasione del Congresso Regionale AIOM Liguria.
La deadline per la domanda di partecipazione è il 31 luglio 2018.
