Questo lavoro pubblicato sull’European Journal of Cancer riporta le raccomandazioni espresse dai panelists delle Linee Guida AIOM “Neoplasie della Mammella” in merito all’ottimizzazione della terapia ormonale adiuvante per pazienti con carcinoma mammario in premenopausa. Il paper riporta per esteso il procedimento secondo la metodologia GRADE che ha portato a formulare le raccomandazioni in merito a 3 quesiti specifici: 1) Nelle donne in premenopausa con carcinoma mammario operato, recettori ormonali positivi, HER2 negativo, a basso rischio, è raccomandabile l’aggiunta della soppressione ovarica al tamoxifene? (raccomandazione: Negativa Debole); 2) Nelle donne in premenopausa con carcinoma mammario operato, recettori ormonali positivi, HER2 negativo, ad alto rischio, è raccomandabile l’aggiunta della soppressione ovarica al tamoxifene? (raccomandazione: Positiva Forte); 3) Nelle donne in premenopausa con carcinoma mammario operato, recettori ormonali positivi, HER2 negativo, ad alto rischio, e candidate a ricevere soppressione ovarica, il trattamento con inibitore dell’aromatasi è raccomandabile rispetto a quello con tamoxifene? An interdisciplinary consensus on the management of bone metastases from renal cell carcinoma
Bone is a major site of haematogenous tumour cell spread in renal cell carcinoma (RCC), and most patients with RCC will develop painful and functionally disabling bone metastases at advanced disease stages. Gli autori hanno provato a proporre delle raccomandazioni per il trattamento della metastasi ossee da tumore del rene. L’approccio multidisciplinare è essenziale per massimizzare il beneficio e dovrebbe esserci un’adeguata gestione del dolore assicurato in tutti i pazienti. La chirurgia è un approccio curativo in pazienti selezionati. Se indicato, la chirurgia deve essere eseguita prima di iniziare il trattamento farmacologico sistemico. Notevoli progressi tecnologici sono stati fatti con la radioterapia, come SBRT e radiochirurgia stereotassica. La terapia sistemica più attiva per il trattamento delle metastasi ossee deve ancora essere definita. Dati sull’uso di trattamenti osteoprotettivi (difosfonati e denosumab) in pazienti con carcinoma renale metastatico rimangono insoddisfacenti e la combinazione di questi con le terapie antitumorali mirate può provocare un incremento delle tossicità, talora anche debilitanti come l’osteonecrosi della mandibola.

Molti pazienti con adenocarcinoma del colon avanzato rimane in buone condizioni generali al momento della progressione dalla seconda/terza linea e la ricerca di una terapia efficace di linea successiva resta oggi un unmet need. Lo studio FRESCO, un trial di fase 3 randomizzato condotto interamente in 26 ospedali cinesi, ha randomizzato 2:1 oltre 400 pazienti con adenocarcinoma del colon in stadio IV pretrattati con almeno due linee di terapia a ricevere fruquintinib – un potente inibitore di VEGFR 1, 2 e 3 somministrato per os a 5 mg/die (21 gg on e 7 giorni off) ovvero placebo. Lo studio ha raggiunto il suo endpoint primario riportando un vantaggio in sopravvivenza globale di circa 2,6 mesi. Tuttavia, alcune osservazioni riguardo al lavoro sono opportune. Innanzitutto il trial è stato interamente condotto in una popolazione cinese e i risultati non sono quindi applicabili su scala internazionale, dove sono disponibili in questo setting di pazienti almeno altre due molecole (regorafenib e tipiracil-trifluridina) con studi di fase 3 randomizzati. In secondo luogo, i pazienti erano stati complessivamente poco pretrattati: solo il 30% avevano precedentemente ricevuto un VEGF inibitore e meno del 20% una terapia con EGFR inibitore. Infine non è chiaro se si vorrà proseguire anche in area occidentale lo sviluppo dei nuovi antiangiogenici (apatinib, fruquintinib).

Gli autori riportano la loro esperienza nel trattamento con radioterapia stereotassica di 82 pazienti con carcinoma dell’ovaio metastatico che avevano ricevuto una mediana di 3 precedenti linee di trattamento. Una risposta completa è stata riportata nel 60% delle lesioni valutabili, e risposte parziali e stabilizzazioni di malattia nel 16% dei casi circa, con un disease free interval mediano di 7,4 mesi. Nessuana tossicità di grado 3-4 è stata registrata. Lo studio dimostra la fattibilità del trattamento di radioterapia stereotassica nella malattia oligometastatica ovarica, fermo restando una accurata selezione delle pazienti da sottoporrre alla stessa.

Recentemente pubblicate su Annals of Oncology, le linee guida dell’ESMO dedicate al trattamento della diarrea nei pazienti oncologici offrono un’aggiornata carrellata su questo effetto collaterale, tradizionalmente comune nei pazienti sottoposti a trattamento chemioterapico, ma possibile anche nei pazienti che ricevono farmaci a bersaglio molecolare, immunoterapia, terapia ormonale. Il documento sottolinea l’elevato impatto sulla qualità di vita e sulla vita sociale della diarrea, oltre che le possibili complicanze metaboliche. Gli autori evidenziano le differenze nella patogenesi della diarrea associata alle varie terapie, enfatizzando che la prevenzione delle complicanze rappresenta uno dei pilastri della gestione.

La Sezione Regionale AIOM Liguria ha istituito il Premio ‘Giovanni Gardin’ 2018, riservato a Giovani Oncologi di età non superiore ai 40 anni (nell’anno solare in corso), quale riconoscimento per i migliori progetti su argomenti a cui il dottor Giovanni Gardin ha dedicato parte della propria attività clinica.
I premi saranno due, uno dedicato a un progetto su ‘I Tumori Cerebrali in Liguria’ e l’altro dedicato a un progetto sulla ‘Identificazione di Fattori Prognostici/Predittivi nei Tumori Solidi in Setting Adiuvante o Avanzato’. A ciascuno dei due migliori progetti di studio presentati sarà assegnato un premio pari a 4.000 Euro (al lordo della ritenuta fiscale). I premi saranno assegnati in occasione del Congresso Regionale AIOM Liguria.
La deadline per la domanda di partecipazione è il 31 luglio 2018.
