Il trial SWITCH è uno studio “proof of concept” che per la prima volta valuta in modo prospettico l’attività antitumorale dell’effettuazione di un cambiamento della terapia steroidea (dallo standard prednisone 5 mg x 2 a desametasone 0,5 mg die) concomitante ad abiraterone acetato (AA), necessaria per prevenire gli effetti collaterali dovuti all’eccesso di mineralcorticoidi. Alcuni pregressi dati di letteratura suggeriscono infatti che il desametasone abbia una attività antitumorale superiore al prednisone nei pazienti affetti da carcinoma della prostata resistente alla castrazione. Il cambiamento dello steroide a progressione dalla terapia con AA + prednisone è stato riconosciuto come valida opzione terapeutica in pazienti selezionati nell’ambito della Consensus Conference di St. Gallen del 2017 (Eur Urol. 2018 Feb;73(2):178-211. doi: 10.1016/j.eururo.2017.06.002. Epub 2017 Jun 24), nonostante la mancanza, fino ad oggi, di dati prospettici sull’argomento. Lo studio, condotto in 4 centri spagnoli, ha arruolato 26 pazienti in progressione biochimica o in progressione radiologica limitata, in assenza di peggioramento clinico, dopo almeno 12 settimane di terapia con AA (sia nel setting pre che post-docetaxel). Con lo switch a desametasone si sono osservate delle risposte biochimiche durature (riduzione del PSA > 30% nel 46,2% dei pazienti), due risposte radiologiche e molte stabilizzazioni di malattia senza ulteriori effetti collaterali aggiuntivi. Questi primi dati promettenti saranno ulteriormente valutati in un futuro studio randomizzato di fase II che includerà un gruppo di controllo nel quale AA + prednisone viene continuato fino a progressione radiologica.

In questo lavoro viene toccato un punto chiave del trattamento dei tumori renali metastatici, ossia la necessità della nefrectomia citoriduttiva e le tempistiche da utilizzare prima di iniziare un trattamento sistemico con inibitori del VEGFR, alla luce dei recenti dati dello studio CARMENA presentati allo scorso ASCO. Viene fatto un update delle linee guida della società europea di urologia ampliando le raccomandazioni: per i pazienti definiti poor prognosis si conferma che la nefrectomia citoriduttiva sia di scarso beneficio, nonostante vada considerato l’impatto psicologico sui pazienti per la scelta di non rimuovere il tumore primitivo. Nei pazienti a prognosi intermedia rimane comunque da discutere anche la possibilità di differire tale intervento.

In questo lavoro, già presentato dal gruppo di ricercatori francesi ad eventi internazionali, la condizione di iperprogressione appare più frequente nei pazienti affetti da tumore polmonare non a piccole cellule (NSCLC) se trattati con immunocheckpoint inibitori piuttosto che a seguito di monochemioterapia. Si tratta di una valutazione retrospettiva su più di 400 casi consecutivi trattati in seconda/terza linea con monochemioterapia o con PD-1/PD-L1 inibitori in 8 centri nel periodo fra novembre 2012 e aprile 2017. Per iperprogressione vengono intesi quei casi che alla prima valutazione TAC hanno una crescita tumorale superiore al 50. Questa condizione viene descritta nel 13,8% dei pazienti trattati con PD-1/PD-L1 inibitori (con una percentuale di pseudoprogressione pari al 4,7%), essendoci in questo gruppo più pazienti con più di due localizzazioni metastatiche (62,5% vs 42,6%; p = 0,006). La sopravvivenza mediana risulta inferiore in pazienti con iperprogressione entro le prime 6 settimane di terapia, rispetto ai pazienti con una progressione “standard” (3,4 mesi vs 6,2 mesi; hazard ratio 2,18; p = 0,003). Tra i pazienti trattati con monochemioterapia la percentuale di iperprogressione è pari al 5,1% e la sopravvivenza mediana uguale a 4,5 mesi vs 3,9 mesi in quei pazienti con una progressione “standard” alla prima rivalutazione (p = 0,60). Sicuramente l’identificazione precoce (anche su base molecolare) di questi pazienti, potrà meglio guidare la scelta terapeutica qualora l’indicazione in seconda o terza linea sia verso un trattamento immunoterapico.

Negli ultimi anni, vari studi hanno dimostrato l’utilità dell’impiego dei patient-reported outcomes (PROs) non solo nella sperimentazione clinica, ma quale parte integrante della pratica clinica in oncologia. Qualche studio ha addirittura suggerito un beneficio in aspettativa di vita per i pazienti ai quali venga data la possibilità di riportare in tempo reale i sintomi e gli effetti collaterali clinicamente rilevanti, e molti studi hanno dimostrato e ribadito l’impatto positivo dell’impiego dei PROs sulla qualità di vita. Peraltro, l’adozione dei PROs nella routine clinica pone delle problematiche concrete (scelta degli strumenti, compliance dei pazienti, registrazione elettronica o cartacea, disponibilità di personale dedicato, apparente consumo di risorse, ecc). Il lavoro recentemente pubblicato dal Journal of Oncology Practice riporta i risultati di una survey, condotta negli Stati Uniti, che indagava proprio questi aspetti. La survey era stata inviata ai centri iscritti all’iniziativa Quality Oncology Practice, e i rispondenti sono stati 89, pari al 38% dei centri. Con il possibile bias legato all’ovvio interesse per l’argomento in chi ha risposto alla survey rispetto a chi non ha risposto, il 79% dei rispondenti ha giudicato la raccolta dei PROs nella pratica clinica un’alta priorità, ma una percentuale leggermente inferiore (69%) dichiarava di impiegare effettivamente la raccolta. Gli strumenti impiegati da chi dichiarava di eseguire la raccolta dei PROs coprono vari aspetti, soprattutto lo stress dei pazienti, i sintomi e il feedback / soddisfazione dei pazienti sulla propria gestione clinica. I rispondenti elencavano, in cima agli aspetti problematici, la possibilità che i pazienti non abbiano voglia di compilare questionari, e insufficiente personale per organizzare la raccolta. In aggiunta, chi dichiarava di non raccogliere i PROs sottolineava, come aspetto problematico, la mancata integrazione dei PROs nella cartella clinica elettronica.
