L’analisi pubblicata da Ramsey e colleghi riguarda un aspetto sicuramente importante per la gestione dei pazienti oncologici nella pratica clinica, esaminando la prevalenza di infezione da virus dell’epatite B, da visus dell’epatite C e da HIV nei pazienti alla diagnosi di tumore. Nella casistica presa in considerazione, il 6,5% aveva una pregressa infezione da HBV e lo 0,6% un’epatite cronica da virus B; il 2,4% risultava positivo per HCV e l’1,1% per HIV. In una percentuale non trascurabile di casi la diagnosi di infezione non era precedentemente nota. Naturalmente, essendo i risultati basati su una casistica statunitense, i numeri non possono essere applicati alla nostra realtà. Tuttavia, l’interesse del lavoro sta nel ricordare i quesiti relativi all’opportunità di eseguire screening al momento della valutazione basale, al fine di valutare l’opportunità o meno di eseguire trattamento farmacologico per la riattivazione virale. Randomized Phase 1 Trial of Pembrolizumab with Sequential Versus Concomitant Stereotactic Body Radiotherapy in Metastatic Urothelial Carcinoma
Preclinical data indicate that radiotherapy works synergistically with pembrolizumab, but the effect and toxicity of this combination may depend on radiotherapy timing. In questo studio di fase I, 18 pazienti affetti da carcinoma uroteliale metastatico, sono stati trattati con pembrolizumab in associazione a radioterapia stereotassica in modalità sequenziale o combinata. Eventi avversi di grado 1-2 si sono verificati in 6/9 pazienti trattati in modalità sequenziale e 9/9 pazienti trattati in modalità combinata. Non è stato registrato alcun evento avverso di grado 4-5, un solo evento avverso di grado 3 nel braccio di terapia concomitante. Non si è verificata alcuna tossicità dose-limitante. Sebbene lo studio non fosse stato disegnato per confrontare l’efficacia delle due modalità, è interessante notare come il tasso di risposta secondo RECIST v1.1 delle lesioni non sottoposte a radioterapia è stato dello 0% nel braccio di terapia sequenziale e del 44% nel braccio di terapia combinata. Lo studio conclude che l’associazione di immunoterapia e radioterapia nel carcinoma uroteliale è sicura; la discrepanza tra i due bracci per quanto riguarda il dato di attività anti-tumorale suggerisce come il differente timing possa influenzarne l’attività e questo rappresenta un interessante punto di partenza per studi futuri.

Questo lavoro riporta i risultati di una pooled analysis di dati individuali di 9 studi, per un totale di 2.148 pazienti, sul ruolo prognostico dei linfociti infiltranti il tumore (TILs) nel carcinoma mammario triplo negativo non metastatico. Si tratta dell’evidenza più di valore in questo ambito e rappresenta anche l’espressione di un lavoro di collaborazione importante che da anni viene condotto dal TIL Working Group. I risultati confermano un’associazione significativa e indipendente di valori incrementali di TILs e migliore prognosi dopo trattamento con chemioterapia contenente antracicline. Sulla base di questi dati gli autori hanno inoltre messo a punto un modello prognostico che integra i TILs e le altre caratteristiche clinicopatologiche di rilievo.
