Lo studio rivede retrospettivamente l’impatto della ricerca del profilo molecolare sull’outcome in 767 pazienti con tumore ovarico trattate in 3 anni di cui il 27% affette da tumori rari. La prima considerazione è che i tumori ovarici rari in ginecologia oncologica non sono così rari, la seconda è che scegliere un trattamento legato al profilo mutazionale della paziente è fattibile perché questo non si traduce in un ritardo all’inizio del trattamento rispetto alle pazienti un cui il trattamento è stato effettuato in studi che non necessitavano dell’indagine biomolecolare. La terza considerazione è sull’utilità di questo approccio da un punto di vista dell’outcome in quanto non si sono evidenziate differenze in risposta al trattamento tra le pazienti la cui terapia era stata scelta sulla base del profilo molecolare vs le altre, sebbene all’interno dei tumori sierosi di basso grado 4 risposte parziali sono state identificate nelle 13 pazienti trattate con MEK inibitori sulla base del profilo molecolare. Credo che questo ultimo aspetto (l’impatto sull’outcome) necessiti di trial prospettici e randomizzati.

Nei pazienti oncologici liberi da malattia a distanza di anni dalla diagnosi, bisogna discutere anche l’aspetto del rischio di secondi tumori che potrebbero essere diagnosticati nel tempo. In particolare, gli autori dello studio recentemente pubblicato da Lancet Oncology, supportato dal Cancer Research UK, hanno analizzato l’incidenza di altri tumori in pazienti nei quali era stato diagnosticato un tumore in giovane età. La pubblicazione presenta i risultati del Teenage and Young Adult Cancer Survivor Study, uno studio di coorte di popolazione, basato sui dati di oltre 200.000 pazienti, che avessero precedentemente ricevuto una diagnosi di cancro in un’età compresa tra 15 e 39 anni, in Inghilterra o in Galles, nel periodo compreso tra il 1971 e il 2006, e fossero vivi a 5 anni dalla diagnosi. L’analisi ha evidenziato che i gruppi nei quali si è registrata una più elevata incidenza di secondi tumori sono stati i soggetti con prima diagnosi di tumore della mammella, di tumore della cervice, di tumore del testicolo e di linfoma di Hodgkin. Un’elevata percentuale dei casi di secondi tumori “in eccesso” è spiegata da pochi tipi di tumori: l’identificazione di “cluster” di tipi di secondi tumori ha delle ovvie implicazioni nel consentire di identificare le priorità del follow-up a lungo termine. Da sottolineare il dato relativo all’eccesso di successivi tumori del polmone in chi da giovane ha ricevuto una prima diagnosi di altra neoplasia: oltre a fattori di predisposizione genetica, probabilmente un ruolo importante in tale associazione è rappresentato dal rischio legato alla radioterapia ricevuta come parte del trattamento dei primi tumori, nonché all’aumento dell’abitudine al fumo di sigaretta in tali pazienti, con i rischi ad esso associati. Lo studio ha come punto di forza la grande numerosità della casistica analizzata, e si propone come il maggiore studio mai realizzato con l’obiettivo di descrivere i rischi di successivi tumori dopo diagnosi di specifici tipi di tumore in giovane età. Inoltre, come gli autori sottolineano, è il primo studio a descrivere il rischio in eccesso, relativo a specifici tipi di tumori successivi.

Lo studio retrospettivo caso controllo è stato condotto su 689 donne con tumore ovarico e 1.572 controlli sani e documenta una riduzione del rischio di sviluppare un tumore ovarico che si quantifica intorno al 30% nelle donne che hanno allattato al seno per almeno 3 mesi. Il rischio risente in senso migliorativo anche della durata del periodo di allattamento e sul numero degli allattamenti e il beneficio si mantiene per almeno 30 anni. La riduzione del rischio si associa in senso indiretto con la soppressione dei processi ovulatori che sono oggi principalmente implicati nel rischio di tumore della mammella, oltre che con un cambiamento dell’ambiente ormonale che favorisce un clima anti-estrogenico e come tale meno favorevole allo sviluppo della malattia.

FDA issues warning letter to clinic illegally marketing unapproved thermography device, warns consumers to avoid using thermography devices to detect breast cancer
