L’obiettivo dello studio di Conteduca e coll era la valutazione dell’efficacia di docetaxel in relazione all’espressione plasmatica del recettore androgenico, seguito da un’analisi esplorativa volta a definire l’interazione tra il numero di copie del recettore androgenico a livello plasmatico ed il tipo di trattamento (docetaxel e terapie ormonali di nuova generazione). Gli autori hanno dimostrato come un aumento del numero di copie del recettore androgenico a livello plasmatico sia associato ad una ridotta sopravvivenza globale in pazienti con carcinoma prostatico resistente alla castrazione (CRPC) trattati con docetaxel. Inoltre l’analisi esplorativa suggerisce come i pazienti con normale espressione plasmatica del recettore androgenico possano beneficiare maggiormente di terapie ormonali di nuova generazione; al contrario, pazienti con un aumento del numero di copie del recettore androgenico presentino risposte più durature alla chemioterapia con docetaxel. Tali dati suggeriscono come lo stato dei recettori androgenici plasmatici possa rappresentare un potenziale marcatore nella selezione del trattamento di prima linea in pazienti affetti da CRPC, ma necessitano tuttavia di validazione in studi prospettici randomizzati.

Questo lavoro riporta risultati importanti potenzialmente practice-changing che validano la mancata risposta patologica completa (pCR) dopo terapia preoperatoria per neoplasia HER2+ come guida su cui basare la scelta del trattamento adiuvante successivo. Il vantaggio in sopravvivenza libera da malattia a favore del T-DM1 rispetto alla prosecuzione di trastuzumab adiuvante è di entità relativa ed assoluta importante. Non viene evidenziato un vantaggio in sopravvivenza globale, ma il follow-up è breve e si evidenzia una differenza che non raggiunge la significatività statistica. Il trattamento della malattia HER2+ si arricchisce quindi di una nuova opzione, contribuendo a realizzare l’obiettivo di una modulazione dell’intensità del trattamento in funzione del rischio individuale di recidiva. Ovviamente vi è ancora ampio margine di miglioramento per quanto riguarda la capacità di stratificare i pazienti a diversa prognosi. Oltre alla pCR, la valutazione del residual cancer burden, del sottotipo molecolare intrinseco e dei linfociti infiltranti il tumore sono tra i biomarcatori prognostici più promettenti per il carcinoma mammario precoce HER2+.

Sebbene il trattamento di prima linea del carcinoma renale metastatico sia in rapida evoluzione, per l’arrivo delle nuove combinazioni (immuno-checkpoint inibitori ed immuno-checkpoint inibitori + inibitori tirosinchinasici), oggi in Italia gli inbitori tirosinchinasici del VEGR rimangono la scelta primaria per i nostri pazienti. Questo ampio studio real-world (> 600 pazienti) ci conferma come pazopanib sia efficace e sicuro sia nei pazienti con caratteristiche di eleggibilità per un trial clinico che nei pazienti definiti “ineleggibili”. Tale conferma è ancora più importante se pensiamo alla significativa mediana di sopravvivenza libera da progressione e di sopravvivenza globale nel sottogruppo di pazienti a buona prognosi per i quali il trattamento con nivolumab in combinazione ad ipilimumab non sarà disponibile. Infine si conferma la maneggevolezza del farmaco anche nel sottogruppo di pazienti “ineleggibili” per un trial clinico senza significative differenze in termini di interruzioni o riduzione di dose per tossicità.
