Nonostante i notevoli progressi scientifici e le implementazioni terapeutiche registrate negli ultimi 15 anni, il tessuto resta per il tumore polmonare un “problema” in termini di quantità, qualità, ristretta visione, rispetto all’eterogeneità di malattia, archiviazione, ottimizzazione … ponendo quindi sempre la biopsia liquida come un miraggio che si vorrebbe divenisse realtà. Con una tecnica di ultra-deep next-generation sequencing gli autori valutano 37 geni, le cui alterazioni sono frequentemente descritte per il tumore polmonare, in 127 pazienti in stadio avanzato di malattia al momento della diagnosi. Questa popolazione, sulla base dell’analisi tissutale, è suddivisibile in 3 gruppi di pazienti: 91 con una mutazione “driver” o responsabile di una resistenza; 19 senza alcuna alterazione molecolare e 17 nei quali il tessuto non era disponibile e/o sufficiente per un’analisi. Rispetto a questa suddivisione l’indagine su biopsia liquida veniva fatta in cieco. Dei 91 pazienti del primo gruppo (per analisi tissutale) un’alterazione “driver” viene rilevata su biopsia liquida in 68 casi (veri positivi 75%) e nel secondo gruppo per nessun caso è stata rilevata un’alterazione molecolare (veri negativi 100%). Nel terzo gruppo, alterazioni “driver” sono state descritte in 4 casi. Tra le alterazioni descritte (su tessuto e su biopsia liquida): EGFR, KRAS, ALK, ROS1, BRAF, HER2, RET, MET. In considerazione di questi dati, la biopsia liquida potrebbe essere presa in considerazione come indagine complementare a quella tissutale e, tenendo presente la sua specificità, valutata come prima indagine per guidare i passaggi (le biopsie) successive.
Preventive drugs in the last year of life of older adults with cancer: Is there room for deprescribing?

L’articolo recentemente pubblicato da Morin e colleghi su Cancer porta alla nostra attenzione un aspetto spesso trascurato, vale a dire il numero elevato di farmaci per patologie concomitanti che vengono assunti dai pazienti oncologici, specialmente anziani, ma che spesso, in particolare negli ultimi mesi di vita, potrebbero essere interrotti. Analizzando una numerosa casistica di oltre 150.000 pazienti oncologici svedesi, con età media di 81 anni, gli autori hanno evidenziato che il numero medio di farmaci aumenta inevitabilmente nel corso dell’ultimo anno di vita. Molti di tali farmaci sono ovviamente legati alla terapia di supporto per i sintomi e gli effetti collaterali dei trattamenti, ma molti rappresentano invece semplicemente la prosecuzione di precedenti prescrizioni, spesso con intento preventivo (anti-ipertensivi, statine, ipoglicemizzanti, antiaggreganti, ecc.). Alcuni di tali farmaci potrebbero essere sospesi o almeno rivalutati, con potenziale beneficio in termini di costi ma anche di rischio di interazioni farmacologiche.
ALK Resistance Mutations and Efficacy of Lorlatinib in Advanced Anaplastic Lymphoma Kinase-Positive Non–Small-Cell Lung Cancer

Gli inibitori di ALK nel trattamento del NSCLC in stadio avanzato sono diversi (approvati e rimborsati dal SSN o disponibili in programmi differenti di fornitura del farmaco), ma la sequenza terapeutica che ad oggi viene applicata per questi pazienti è assolutamente empirica nonostante, da un punto di vista preclinico, molte siano le informazioni riguardanti i meccanismi di resistenza agli inibitori di prima e seconda generazione. In questo studio Alice Shaw e colleghi descrivono una maggiore attività di lorlatinib nei casi in cui la resistenza sia mediata da mutazione di Alk rispetto ad un diverso meccanismo di resistenza. Su 198 pazienti inseriti nel trial registrativo di fase II di lorlatinib, in stadio IV affetti da NSCLC con riarrangiamento di ALK viene valutato il profilo molecolare su plasma attraverso Guardant360 e l’analisi viene parimente condotta su tessuto. Il 24% dei pazienti rivela la presenza di mutazioni resistenti di Alk su plasma o su tessuto. Nei pazienti con malattia resistente a crizotinib, la risposta obiettiva era pressochè la stessa nei pazienti con differente genotipo, in cui la resistenza veniva valutata su plasma o su tessuto, mentre nei pazienti trattati con inibitori di seconda generazione la risposta risultava differente nei pazienti con o senza mutazione di Alk testata su plasma (62% vs 32%) e su tessuto (69% vs 27%), cosi come risultavano emergere differenze in termini di sopravvivenza libera da progressione nei pazienti con o senza mutazione di Alk testata su tessuto (mediana = 11,0 vs 5,4 mesi; HR = 0,47; IC 95%: 0,27–0,83), essendo invece pressochè la stessa quando testata su plasma. Il dato necessita sicuramente di maggiori dettagli e dell’implementazione della rivalutazione di malattia anche nella pratica clinica per meglio utilizzare le opzioni terapeutiche a nostra disposizione.
