Il lavoro pubblicato su European Urology da Sternberg e coll. descrive i risultati preliminari dello studio SAUL, uno studio multicentrico a braccio singolo volto a valutare sicurezza ed efficacia di atezolizumab in uno scenario real-world internazionale. Lo studio ha arruolato 1.004 pazienti affetti da carcinoma uroteliale o non uroteliale delle vie urinarie, in progressione dopo almeno una linea di trattamento sistemico per malattia metastatica. Lo studio prevedeva il trattamento anche in pazienti con contrazione della funzionalità renale, metastasi encefaliche trattate e malattie autoimmuni. Il 45% dei pazienti ha presentato almeno una tossicità di grado uguale o superiore a 3 e tra questi i più frequenti sono stati la fatigue, l’astenia, le coliti e l’ipertensione. La sopravvivenza mediana è stata di 8,7 mesi, la sopravvivenza libera da progressione mediana di 2,2 mesi. Considerando solo i pazienti con le medesime caratteristiche dei pazienti arruolati nello studio di fase III IMvigor 211 (che ha portato all’approvazione di atezolizumab da parte dell’FDA e dell’EMA in seconda linea dopo progressione a terapia a base di platinoide), ed escludendo pertanto i pazienti con ECOG PS 2, con malattie autoimmuni, metastasi encefaliche, istologia non uroteliale o contrazione della funzionalità renale, la sopravvivenza globale è risultata essere di 10 mesi, in linea con i dati degli studi precedenti. Nonostante la popolazione trattata fosse molto meno selezionata rispetto a quella dei precedenti studi il farmaco ha presentato un buon profilo di tollerabilità e l’efficacia è stata confermata. Lo studio ha quindi fornito informazioni utili sulla tollerabilità del farmaco in sottogruppi di pazienti per i quali non esistevano dati, come i pazienti con PS 2, metastasi encefaliche, contrazione della funzionalità renale e con malattie autoimmuni in buon controllo.

Lo studio analizza una casistica retrospettiva di 21 pazienti con recidiva di carcinoma ovarico con metastasi cerebrali e ne analizza il comportamento clinico. La metastasi cerebrale è un evento raro nel carcinoma ovarico, ma sembra che le pazienti con la mutazione BRCA abbiano una incidenza aumentata. Lo studio documenta che la metastasi cerebrale rimane un evento che segna la prognosi con una sopravvivenza globale post-diagnosi di 9 mesi, ma alcuni fattori prognostici sono stati identificati e credo siano molto utili nel counseling con la paziente: l’intervallo libero da malattia tra la diagnosi primaria e e la diagnosi di metastasi cerebrale, l’aver effettuato una asportazione chirurgica completa della lesione e la successiva radioterapia.

La cessazione tabagica ha notoriamente una sua rilevanza non solo prima di una diagnosi oncologica, ma anche dopo, sia per ottenere maggiore compliance, tollerabilità ed efficacia terapeutica, sia per ridurre le possibilità di recidiva e/o di insorgenza di secondo tumore. In questo lavoro, vengono valutati i costi associati al fallimento della prima linea terapeutica in pazienti oncologici che continuino a fumare con l’impiego di un modello statistico che include la descrizione dei tassi di fallimento terapeutico (in prima linea) in pazienti non fumatori, la prevalenza dei fumatori, l’ORR in fumatori e non fumatori e i costi del trattamento post-fallimento terapeutico dalla prima linea. Con un atteso fallimento terapeutico pari al 30% nella popolazione di non fumatori, una prevalenza di fumatori pari al 20% e un atteso 60% aumento di fallimento terapeutico nei fumatori (OR=1,6) e un costo medio di $100.000 per il trattamento dei pazienti dopo fallimento da prima linea, il costo aggiuntivo per 1.000 pazienti era di $2,1 milioni, ossia un costo addizionale medio per paziente fumatore pari a $10.678. Il che corrisponde a $3,4 miliardi in termini di costi addizionali se si stimano 1,6 milioni di pazienti annualmente diagnosticati con patologia oncologica.
