I pazienti con un precedente trapianto d’organo sono tra le categorie “speciali” per le quali la decisione relativa all’eventuale trattamento antitumorale con farmaci immune checkpoint inhibitors è molto delicata. Tali pazienti, infatti, non sono rappresentati negli studi registrativi di tali farmaci, per i quali l’anamnesi di trapianto d’organo rappresentava un criterio di esclusione, e la decisione di candidare il paziente al trattamento immunoterapico è combattuta tra la volontà di offrire un’opportunità terapeutica efficace e il rischio di tossicità, che nel caso specifico può comportare il rigetto del trapianto. In uno scenario che si basa su evidenze deboli, Ros e colleghi sintetizzano, nella recente pubblicazione di Annals of Oncology, la letteratura esistente sull’argomento, basata essenzialmente sul report di casi clinici. Dalla sintesi dei casi esistenti in letteratura, emerge che il trattamento immunoterapico, con farmaci anti-CTLA-4 o anti-PD-1, è associato a un rischio tutt’altro che trascurabile di rigetto. In particolare, gli autori evidenziano che il rischio appare particolarmente elevato con i farmaci anti-PD-1, e molto meno con i farmaci anti-CTLA-4, lasciando ipotizzare che il diverso meccanismo d’azione delle 2 categorie di farmaci esponga a un diverso rischio di rigetto del trapianto. Nel caso dei farmaci anti-PD-1, il rigetto si era manifestato dopo un intervallo mediano relativamente breve rispetto all’inizio del trattamento (28 giorni, con un range da 5 a 120). Con tutti i limiti di una raccolta di questo tipo, non sembra che l’intervallo di tempo trascorso dal trapianto d’organo all’inizio dell’immunoterapia rappresenti un fattore predittivo del rischio di rigetto, essendo risultato simile nei casi con rigetto rispetto a quelli che non avevano manifestato il problema. In conclusione, si tratta di un dato da tener presente nel valutare i rischi e i benefici dell’eventuale immunoterapia: in molti casi, nonostante l’avvio tempestivo della terapia immunosoppressiva “di salvataggio”, l’evento avverso si è concluso con il rigetto definitivo dell’organo trapiantato.

In un momento storico in cui le Società Scientifiche e le Istituzioni stanno valutando la necessità e fattibilità di implementare un valido metodo di screening per il carcinoma polmonare, può essere interessante vedere i risultati di uno studio che esamina il trend delle discussioni fra medico e paziente in merito allo screening per carcinoma polmonare, facendo anche delle correlazioni con la volontà o meno di smettere di fumare. In generale lo studio evidenzia, a decorrenza dal 2012 (si ricorda che i dati sullo screening del tumore polmonare dello studio statunitense sul New England Journal of Medicine sono del 2011) una riduzione delle discussioni in merito e soprattutto che queste non hanno alcun legame con la volontà di smettere o meno di fumare. La raccomandazione ad eseguire il programma di screening secondo i criteri pubblicati sul New England Journal of Medicine sono da datarsi 2013, con una precisa policy ad opera del Centers for Medicare & Medicaid Services risalente al 2015. Lo studio pubblica i dati di una survey telefonica che ha collezionato informazioni in ambito sanitario nel 2012, 2014 e 2017 nella popolazione adulta. Nella survey venivano chieste informazioni in merito a possibili colloqui avutisi fra i soggetti intervistati ed il loro medico di base a riguardo dell’habitus tabagico e sullo screening per il tumore polmonare. Veniva inoltre chiesto agli intervistati in merito alla loro volontà di smettere di fumare/tentativi nella cessazione. L’analisi delle risposte è stata effettuata considerando anche caratteristiche quali il sesso, l’età, lo stato sociale, la copertura assicurativa. Su 9.443 soggetti intervistati, un colloquio con il proprio medico riguardo allo screening per tumore polmonare si riduce dal 6,7% (valore già di per sé molto basso) nel 2012 al 4,2% nel 2014 e 4,3% nel 2017. Guardando nello specifico alla suddivisione per età ed abitudine tabagica i fumatori attivi e in età fra i 55 e 74 anni dichiaravano, per esempio, di aver avuto questo tipo di colloquio nel 26,8% dei casi nel 2012 e i valori più alti in anni successivi si registravano nei fumatori attivi con più di 74 anni (23,5% nel 2014; 22,1% nel 2017). Le categorie che, in generale, riportavano una maggiore percentuale di colloqui al riguardo erano i neri non-ispanici, gli ispanici, gli individui con copertura assicurativa e i pazienti già con patologia cardiaca o polmonare diagnosticata. Dall’analisi multivariata emerge come la discussione a tal proposito fra soggetto e medico curante non aveva alcuna associazione con la volontà di smettere di fumare o i tentativi di cessazione in un’adeguata finestra temporale. Lo studio ha sicuramente enormi limiti interpretativi, ma è utile per capire come sia assolutamente necessario, insieme a tutti gli accorgimenti e decisioni che verranno prese su come, dove e con quali figure implementare lo screening in Italia, sarà assolutamente indispensabile includere anche un adeguato training dei medici di base per sfruttare al meglio il loro ruolo cruciale con il paziente, soprattutto per uno screening che non potrà essere attivo come quelli al momento in essere in Italia.

I pazienti affetti da tumore del polmone sono frequentemente anziani e spesso, già al momento della diagnosi, hanno sintomi di malattia che rendono necessaria la presenza di una o più caregivers. Tale assistenza si avvale, in alcuni casi, di personale pagato, ma spesso ricade sui parenti del paziente (spesso il coniuge e/o i figli), che si prendono cura del paziente, sia per quanto riguarda le necessità al proprio domicilio, sia per quanto riguarda gli accessi in ospedale. Questo impegno può comportare, per i caregivers, anche importanti sacrifici al lavoro, o nelle altre attività quotidiane, e contribuisce sicuramente alla “tossicità finanziaria” di cui ultimamente si parla anche in Italia. L’articolo pubblicato da Supportive Care in Cancer riporta i risultati di una survey condotta tra il 2015 e il 2016 in Italia, Germania e Francia, coinvolgendo i pazienti affetti da tumore del polmone non a piccole cellule (NSCLC) avanzato, ed i loro caregivers. Nel dettaglio, l’analisi è stata condotta su una popolazione di 427 caregivers e i corrispondenti 427 pazienti, la maggior parte dei quali stava ricevendo una terapia di prima linea per il tumore del polmone avanzato. Il carico complessivo settimanale del supporto offerto dal caregiver è stato quantificato, mediamente, in 29,5 ore per settimana. Tra le tipologie di supporto dichiarate dai caregivers c’erano, ad esempio, l’accompagnamento al lavoro o agli appuntamenti in ospedale (65,9% in Italia), l’aiuto a preparare i pasti (42,6% in Italia), l’aiuto a fare la spesa (37,2% in Italia), l’aiuto nel prendere i farmaci (43,4% in Italia) e numerose altre attività, con un impatto negativo complessivo sull’attività lavorativa, per i caregivers occupati, che è risultato pari al 21,1% in Germania, al 30,4% in Francia e al 29,7% in Italia. Per definire i complessi e delicati aspetti indagati nell’analisi relativamente al “carico” dei caregivers dei pazienti affetti da tumore del polmone, gli autori hanno usato il termine “humanistic burden”, che sottolinea quanto tale carico comprenda sia aspetti psicologici, sia aspetti concreti legati all’attività lavorativa e alle altre attività quotidiane. Vale la pena di sottolineare che, in Italia, la maggior parte del carico ricade spesso sul nucleo familiare del paziente, e questo ha importanti ricadute, oltre che psicologiche, anche concrete in termini di disagi quotidiani, in particolare per chi lavora. Sia quest’ultimo aspetto, sia l’eventuale necessità di ricorrere all’assistenza di caregivers pagati, concorrono in maniera potenzialmente importante alla “tossicità finanziaria”, che non è solo legata ai costi diretti dei farmaci antitumorali (che, almeno in Italia, ricadono sul servizio sanitario nazionale), ma a molti altri aspetti, incluse le spese per l’assistenza e le conseguenze negative per il lavoro (del paziente, ma anche delle persone che lo assistono).
