Genomic and transcriptomic profiling expands precision cancer medicine: the WINTHER trial

Viene valutata la potenzialità del considerare il profilo basato sull’analisi dell’RNA (trascrittomica) unitamente a quello del DNA per una migliore applicazione del concetto di medicina di precisione in oncologia in pazienti affetti da tumore solido in stadio avanzato in progressione dopo terapia standard. I pazienti nello studio WINTHER venivano prima valutati per alterazioni mutazionali targettabili e quelli per i quali queste alterazioni non erano identificabili, utilizzando un preciso algoritmo diagnostico sviluppato dal WIN Consortium. Sono stati arruolati 303 pazienti, fra cui 107 sono i pazienti che rientrano nella valutazione per lo studio WINTHER, tutti pesantemente pretrattati (25% avevano ricevuto 5 o più linee terapeutiche), 69 trattati secondo valutazione del DNA e 38 sulla base dell’RNA profiling. Complessivamente lo studio viene considerato valido per promuovere la terapia personalizzata in pazienti con tumore solido in stadio avanzato (qui applicata nel 35% della popolazione a differenza di altri dati di letteratura che riportano percentuali variabili fra 5 e 25%), essendo in grado di ampliare questo approccio con l’impiego dell’RNA profiling. I pazienti trattati con approccio personalizzato hanno riportato un outcome migliore (se in buone condizioni generali). L’affinarsi delle tecniche diagnostiche (sempre se affiancate da un adeguato sviluppo di nuove molecole e consono accesso ai farmaci o ai trials clinici) porterà ad una più ampia applicazione della medicina di precisione in ambito oncologico con conseguente miglioramento dell’aspettativa di vita per i pazienti.

Analisi come quella pubblicata recentemente da BMJ Open non dicono niente di particolarmente nuovo per gli oncologi (l’analisi documenta l’incremento del prezzo dei farmaci oncologici, in particolare quelli impiegati per il tumore del colon-retto, nell’arco di tempo compreso tra il 1981 ed il 2015, nel Regno Unito). L’elemento interessante è che il dato non si basa sull’analisi dei soli farmaci oncologici, ma prende in considerazione farmaci di 5 aree terapeutiche (ipertensione, asma, artrite reumatoide, schizofrenia e appunto il tumore del colon-retto). Come è facile immaginare, il costo medio dei farmaci oncologici supera quello delle altre categorie, in particolare per i farmaci biologici. Deve far riflettere il fatto che questi articoli, pubblicati su una rivista non oncologica, portano all’attenzione della comunità medica il problema del costo dei farmaci per il cancro, più o meno velatamente additandoli come responsabili di un marcato incremento della spesa farmaceutica, e anche mettendo in dubbio il reale valore di tale spesa. Nelle conclusioni, infatti, gli autori dicono: “il prezzo dei farmaci continua a salire, trasversalmente in diverse aree terapeutiche. Questo ha qualche associazione con le novità, ma non è chiaro se l’incremento dei prezzi sia associato a benefici clinici”. Si parla di “novelty”, che letteralmente vuol dire “novità” e non necessariamente “innovazione”. In un momento storico in cui si avverte in maniera crescente la limitatezza delle risorse, è probabile che possa crescere l’accusa di scarso valore clinico per alcune spese in ambito oncologico, non solo da parte del pagatore (ad esempio il servizio sanitario), ma anche da parte degli altri specialisti che concorrono alla divisione delle risorse. Per questo, è particolarmente importante che gli oncologi siano i primi a valutare criticamente l’innovatività e il valore dei trattamenti, al fine di difendere con successo quelli che veramente valgono.

Lo studio ha retrospettivamente analizzato i dati di efficacia e sicurezza di niraparib nella popolazione anziana dello studio NOVA sul mantenimento nella recidiva platino-sensibile di tumore ovarico. Il farmaco si conferma come estremamente efficace nell’aumentare la sopravvivenza libera da progressione sia nella coorte di pazienti con mutazione del BRCA sia nella coorte senza mutazione, rispetto al placebo, ma il dato più interessante è che non emergono elementi di safety nella pazienti anziane diversi o più gravi che nelle pazienti con < 70 anni con la tossicità ematologica che si conferma come la più frequente. Il dato è interessante perché la popolazione non mutata è costituita da pazienti anziane e mediamente post-menopausali e questo dato ci conforta rispetto alla possibilità di utilizzare il farmaco in sicurezza. Recentemente all’SGO sono stati presentati i dati del rucaparib, un altro PARP inibitore, nelle pazienti anziane e una maggiore incidenza di rinvii di trattamento e riduzioni di dose è stata registrata nelle pazienti anziane, pur non emergendo anche in questo caso nuove eventi di safety per il farmaco nella popolazione più anziana.
