L’aumento di prevalenza di alcune patologie oncologiche è indubbiamente il segnale di un avanzamento della ricerca in ambito oncologico, che porta con sè una serie di nuovi approcci e modus operandi che vanno riservati a questa fetta della popolazione. Le localizzazioni cerebrali sono sede comune di metastatizzazione per molte malattie tumorali e in questa pubblicazione viene affrontata la tematica di questa sede di ripresa di malattia, anche a distanza di molti anni dall’interruzione delle cure, in popolazione affetta da carcinoma mammario, polmonare e melanoma. Viene preso in considerazione proprio il dato di aumento di sopravvivenza, che può significare un dilatarsi dei tempi con maggiore probabilità di sviluppo di metastasi cerebrali e, conseguentemente, necessità di effettuare controlli mirati in tale distretto. Per le tre patologie sopra indicate vengono valutate le localizzazioni cerebrali in tempistiche diverse della malattia (dal momento dello staging fino a parecchio tempo dopo) in una casistica comprendente 70.974 casi di tumore polmonare, 67.362 di carcinoma mammario e 21.860 melanomi. Nel caso del polmone, le localizzazioni cerebrali sincrone sono state descritte nel 9,6% dei casi e successivamente nel 13,5% dei casi. Per il carcinoma mammario, rispettivamente nel 0,3 e 1,8%. Nel caso del melanoma, rispettivamente nell’1,1% e 3,6% (con ampie variabilità nei casi di malattie esordite come metastatiche, coinvolgimento linfonodale locale e non e altre caratteristiche di malattia). Di per sé lo studio non presenta nessun dato eclatante e/o non noto per le patologie esaminate, ma solleva la problematica di come (e quando) approcciare i pazienti oncologici in “lungo follow-up” proponendo differenziazioni basate sull’età e/o sulle caratteristiche delle singole patologie.
Nintedanib in combination with pemetrexed and cisplatin for chemotherapy-naive patients with advanced malignant pleural mesothelioma (LUME-Meso): a double-blind, randomised, placebo-controlled phase 3 trial
Nintedanib targets VEGF receptors 1–3, PDGF receptors and , FGF receptors 1–3, and Src and Abl kinases, which are all implicated in malignant pleural mesothelioma pathogenesis. A fronte di risultati positive e promettenti dell’antecedente studio di fase II, questo studio randomizzato non conferma il vantaggio dell’aggiunta di nintedanib a cisplatino e pemetrexed (rispetto alla sola doppietta chemioterapica) nel trattamento del mesotelioma pleurico in stadio avanzato. Dall’aprile 2016 a gennaio 2018, 541 pazienti vengono screenati e 458 sono stati randomizzati nello studio LUME-Meso. La durata media di trattamento è stata di 5,3 mesi nel braccio con nintedanib (range: 2,8 – 7,3) e 5,1 (range: 2,7 – 7,8) nel braccio con placebo. Al raggiungimento dei 250 eventi, nessuna differenza viene descritta per la sopravvivenza libera da progressione fra i due bracci di trattamento (mediana: 6,8 mesi per nintedanib [IC 95%: 6,1 – 7,0] e 7 mesi per il placebo [IC 95%: 6,7 – 7,2]; HR 1,01 [IC 95%: 0,79 – 1,30]; p = 0,91). Purtroppo questa è l’ennesima sconfitta in una patologia ad elevato impatto sociale e l’ennesima dimostrazione che anche risultati promettenti negli studi di fase II possono non riflettersi nel dato di fase III.
Local Consolidative Therapy Vs. Maintenance Therapy or Observation for Patients With Oligometastatic Non-Small-Cell Lung Cancer: Long-Term Results of a Multi-Institutional, Phase II, Randomized Study

Con un lungo follow-up di questo studio di fase II, gli autori confermano la superiorità del consolidamento terapeutico con trattamento locale vs terapia di mantenimento o semplice osservazione in malattia oligometastatica (NSCLC). Pazienti affetti da NSCLC in stadio avanzato con ≤ 3 metastasi e senza progressione ad almeno 3 mesi (o più) dal termine della prima linea di terapia venivano randomizzati (1:1) a terapia di mantenimento o semplice osservazione (n = 24) o trattamento consolidativo locale con radioterapia o chirurgia di tutti i siti metastatici (n =25). L’arruolamento veniva chiuso dopo l’inserimento di 49 pazienti dato il significativo vantaggio (in termini di sopravvivenza libera da progressione, PFS) nel braccio con trattamento consolidante. Con un follow-up di 38,8 mesi (range: 28,3 – 61,4 mesi), il vantaggio in PFS viene mantenuto (PFS mediana: 14,2 mesi vs 4,4 mesi; p = 0,022). La sopravvivenza globale (OS) mediana risulta di 41,2 mesi nel braccio con consolidamento locale vs 17,0 mesi (p = 0,017). Pur in assenza di una definizione universale e univoca di “malattia oligometastatica” è chiaro che questi dati siano molto interessanti, soprattutto per alcuni sottogruppi di pazienti (vd oncogene addicted) sebbene alcuni segnali di tossicità vadano meglio letti ed interpretati, e probabilmente numerosità superiori potranno meglio dirimere chi di questi pazienti possa trarre un assoluto beneficio da questo approccio.

May 24, 2019 – The FDA has approved ruxolitinib for the treatment of adult and pediatric patients ≥12 years of age with steroid-refractory acute graft-versus-host disease (aGVHD).
