Erdafitinib, potente inibitore tirosin-chinasico di FGFR1-4, in questo studio di fase 2 ha dimostrato di essere assai attivo nei pazienti affetti da carcinoma uroteliale metastatico con alterazione del FGFR, indipendentemente dalle linee di terapia precedentemente effettuate, dal tipo di terapia precedente, dalla presenza di metastasi viscerali e dalla sede di primitività tumorale. Anche le tossicità più frequentemente riscontrate in corso di terapia con erdafitinib tra cui l’iperfosfatemia e gli eventi oculari sono ben gestibili e per lo più di grado lieve-moderato e con la sospensione e la successiva riduzione di dose sono per la maggior parte risolvibili senza sequele importanti. Inoltre non bisogna dimenticare che la durata mediana di sopravvivenza dei 99 pazienti con alterazioni di FGFR, trattati in questo studio e che normalmente non rispondono all’immunoterapia, si avvicina a quella ottenuta con pembrolizumab e atezolizumab. Questo è un risultato incoraggiante in una patologia che fino a pochi anni fa è stata orfana di trattamenti e dove le terapie a bersaglio molecolare personalizzate erano sempre state deludenti.

Molto si è parlato di e-cig negli ultimi giorni, spesso anche in modo non adeguato, spesso solo per “catturare la notizia che fa scalpore” più che per fornire un’informazione fondata e scientificamente corretta. Il principio fondamentale di tutte le campagne di prevenzione primaria dovrebbe essere la protezione delle categorie più deboli e più a rischio (fra cui giovani e bambini) ed evitare le esposizioni secondarie a fattori di rischio noti e/o dubbi. In questa survey nazionale viene chiaramente riportato un aumento nel 2018 (rispetto a dati del 2015-2017) dell’esposizione delle scolaresche ad aerosol di e-cigarettes derivante dalla convivenza con adulti svapatori. L’attenzione va posta su quanto presente nei fumi dell’e-cig (ossia nicotina, metalli pesanti, aldeidi, glicerina, additivi, …) oltre che sull’insegnamento/messaggio che viene trasmesso alle giovani generazioni avvolte in questa nube di svaping (!) e al fatto che unitamente a questa esposizione rimane reale e presente anche quella al “secondhand tobacco smoke”, ossia all’accertata nociva esposizione di seconda mano al fumo di sigaretta.

Il trattamento di prima linea del carcinoma a cellule renali ha visto recentemente l’introduzione di combinazioni di immunoterapie (Ipi-Nivo: ipilimumab e nivolumab) e le combinazioni di immunoterapia e inibitori di VEGF (IOVE: avelumab + axitinib, pembrolizumab + axitinib, bevacizumab + atezolizumab). Non esistendo dati diretti di confronto tra i due tipi di combinazioni, lo studio retrospettivo di Dudani e coll, appena pubblicato su European Urology, fornisce dati interessanti sulla scelta del trattamento di prima linea. Si tratta di uno studio retrospettivo che ha incluso oltre 10.000 pazienti consecutivi affetti da carcinoma a cellule renali dall’International Metastatic Renal-cell Carcinoma Database Consortium, dei quali 188 trattati in prima linea con Ipi-Nivo o IOVE. Lo studio ha dimostrato un tasso di risposta pari al 33% nei pazienti trattati con Ipi-Nivo vs 40% in quelli trattati con IOVE (IC 95%: -8 -22; p = 0,4), il tempo al fallimento del trattamento è stato pari a 14,3 vs 10,2 mesi (p = 0,2), il tempo al trattamento di seconda linea di 19,7 vs 17,9 mesi ( p = 0,4). I dati di sopravvivenza globale (OS), seppur immaturi non hanno mostrato differenze significative tra i due trattamenti (p = 0,17). Il secondo dato interessante dello studio proviene dalla descrizione del trattamento di seconda linea impiegato: il 34% dei pazienti ha ricevuto un trattamento di seconda linea, che nell’88% dei pazienti è stato un inibitore di VEGF (axitinib, cabozantinib, lenvatinib + everolimus, pazopanib o sunitinib). Il tasso di risposta all’inibitore di VEGF di seconda linea è stato inferiore nei pazienti trattati con IOVE versus Ipi-Nivo (15% vs 45%; IC 95%: 3-57; p = 0,04). I limiti dello studio sono rappresentati dalla natura retrospettiva dello stesso e dal limitato numero di pazienti inclusi nell’analisi, soprattutto per quanto riguarda i dati sulla seconda linea dopo IOVE o Ipi-Nivo. Si tratta tuttavia della prima ed al momento unica analisi di confronto tra le due strategie terapeutiche (Ipi-Nivo vs IOVE) in prima linea nel carcinoma renale metastatico, che non ha dimostrato differenze significative in termini di tasso di risposta, tempo al fallimento del trattamento, tempo alla seconda linea e OS. Nella pratica clinica al momento pertanto la scelta non può basarsi su dati di efficacia, ma esclusivamente sulla valutazione del profilo di tossictà dei trattamenti, i costi, la preferenza del medico e del paziente.
