Hype or hope? The strange case of platinum salts’ renaissance in breast cancer
In the last decade, the clinical applicability of platinum salts in breast cancer (BC) has gained momentum, mainly in the context of homologous recombination-deficiency (HRD) where effects of DNA-damaging agents are particularly enhanced. L’utilizzo dei sali di platino nel trattamento del carcinoma mammario sta vivendo un nuovo rinascimento, in particolare nelle forme con deficit dei sistemi di riparazione del DNA e/o in presenza di mutazioni dei geni BRCA 1/2. Sotto l’aspetto clinico, in considerazione del costo e dei risultati ancora controversi di caratterizzazioni molecolari più fini, le pazienti con carcinoma triple-negative, per l’intrinseca instabilità genomica che contraddistingue tale sottogruppo, hanno la maggiore probabilità di trarre beneficio dall’impiego di regimi a base di platino. Particolarmente interessanti, per la sinergia di azione, sono le combinazioni con taxani come dimostrato dallo studio clinico di fase II tnAcity, che ha evidenziato l’efficacia dell’associazione carboplatino/nab-paclitaxel nel trattamento di prima linea del carcinoma mammario avanzato triple-negative. In termini di prospettive future, si guarda con attenzione alla capacità dei sali di platino di indurre la formazione di neoantigeni e di modulare il sistema immunitario, immaginando sviluppi nel campo dell’immunoterapia con significative implicazioni terapeutiche, possibilmente rivoluzionarie.
Olaparib in patients with metastatic castration-resistant prostate cancer with DNA repair gene aberrations (TOPARP-B): a multicentre, open-label, randomised, phase 2 trial

Fino ad oggi, le scelte terapeutiche per i pazienti affetti da carcinoma della prostata candidato a trattamento sistemico non sono state basate sulla caratterizzazione molecolare della neoplasia, a differenza del crescente ruolo delle analisi molecolari in altri tipi di tumori solidi. Qualche mese fa, al meeting ESMO, sono stati presentati i risultati dello studio di fase III PROfound, che confrontava il PARP inibitore olaparib con un agente ormonale di nuova generazione in pazienti affetti da tumore della prostata resistente alla castrazione, che avessero fallito una precedente linea ormonale di nuova generazione (abiraterone o enzalutamide). I pazienti erano selezionati per la presenza di alterazioni dei geni del riparo del DNA mediante ricombinazione omologa (divisi in 2 coorti, la prima comprendente alterazioni di BRCA1, BRCA2 e ATM, la seconda comprendente alterazioni di altri geni). Quello studio aveva evidenziato un vantaggio significativo in sopravvivenza libera da progressione a favore del trattamento con olaparib, rappresentando un’importante tappa verso l’introduzione di una terapia personalizzata in questo setting. Sono ora pubblicati su Lancet Oncology i risultati dello studio TOPARP-B, che ha randomizzato pazienti con tumore della prostata resistente alla castrazione, con la presenza documentata di alterazioni nei geni del riparo, già trattati con 1 o 2 linee di chemioterapia, a ricevere olaparib alla dose di 400 o 300 mg due volte al giorno. Ciascuno dei 2 bracci era disegnato per valutare l’attività del trattamento con il PARP inibitore in termini di risposte (valutate come riduzione dimensionale delle lesioni secondo RECIST, oppure riduzione di almeno il 50% del valore del PSA, oppure riduzione del valore delle cellule tumorali circolanti). Olaparib ha dimostrato attività pari al 54,3% dei pazienti nella coorte trattata alla dose di 400 mg due volte al giorno, e pari al 39,1% nella coorte dei pazienti trattati alla dose inferiore. Da notare che lo screening molecolare aveva identificato alterazione eleggibile per lo studio in 161 pazienti di 711 screenati, pari al 22,6%. Come sottolineato dagli autori nelle conclusioni, si tratta probabilmente di un altro tassello verso l’adozione della caratterizzazione molecolare e verso l’impiego di terapie personalizzate nella futura pratica clinica.
IMbrave150: Efficacy and safety results from a ph III study evaluating atezolizumab (atezo) + bevacizumab (bev) vs sorafenib (Sor) as first treatment (tx) for patients (pts) with unresectable hepatocellular carcinoma (HCC)

Dal 2008, la terapia medica di prima linea del carcinoma epatocellulare non resecabile (uHCC) è rappresentata dall’inibitore multichinasico sorafenib. Il valore dell’efficacia degli inibitori tirosin-chinasici (TKI) come standard di cura in questa malattia è stato confermato quando un altro farmaco target, il lenvatinib, è risultato non inferiore al sorafenib in termini di sopravvivenza globale (OS). Un recente studio di fase III con nivolumab versus sorafenib ha dato risultati negativi, mettendo in discussione il valore dell’immunoterapia in questa malattia, almeno per i pazienti non selezionati, data la mancanza di un biomarcatore predittivo di risposta. Sorprendentemente, visti i precedenti risultati negativi degli studi di immunoterapia nei pazienti con uHCC, lo studio di fase III IMbrave150, presentato all’ESMO-Asia, ha posto le basi per un nuovo standard di cura nella terapia di prima linea dell’HCC. La combinazione dell’anticorpo anti PD-L1 atezolizumab e dell’anticorpo anti VEGF bevacizumab è risultata infatti superiore rispetto a sorafenib come terapia di prima linea in termini di OS (con un hazard ratio di 0,58) e sopravvivenza libera da progressione (HR 0,59). Il tasso di risposte obiettive è risultato nettamente superiore nel gruppo di combinazione rispetto alla monoterapia, mentre il profilo di tossicità osservato è stato in linea con il profilo dei due farmaci presi singolarmente e non ha mostrato effetti inaspettati. Non è la prima volta che la combinazione di un inibitore di VEGF con un inibitore di immune checkpoint mostra dei risultati positivi, che potrebbero aprire la strada a terapie combinate simili non solo nell’HCC, ma anche in altre patologie, come nel carcinoma del rene e dell’endometrio.
