Olaparib plus Bevacizumab as First-Line Maintenance in Ovarian Cancer

Considerando l’elevato rischio di recidiva delle pazienti con carcinoma ovarico, la ricerca degli ultimi anni è stata rivolta allo studio di terapie di mantenimento, ottenendo risultati entusiasmanti nel ridurre il tempo alla recidiva dapprima con il bevacizumab e successivamente con l’impiego degli inibitori di PARP.
Lo studio PAOLA-1 nasce con l’obiettivo di valutare l’efficacia dell’olaparib in associazione a bevacizumab nell’ipotesi di un effetto sinergico dei due farmaci. È uno studio di fase III, in doppio cieco, che ha confrontato l’efficacia e la sicurezza di olaparib in aggiunta a bevacizumab versus bevacizumab da solo, come trattamento di mantenimento in donne con tumore ovarico sieroso o endometrioide, delle tube di Falloppio o peritoneale, in stadio III-IV di alto grado, in risposta completa o parziale al trattamento in prima linea con chemioterapia contenente platino e bevacizumab, indipendentemente dallo stato mutazionale di BRCA. I risultati hanno mostrato che olaparib in combinazione con bevacizumab ha ridotto il rischio di progressione della malattia o di morte del 41% e migliorato la sopravvivenza libera da progressione (PFS) con una mediana di 22,1 mesi rispetto ai 16,6 mesi nelle pazienti trattate con bevacizumab in monoterapia (HR 0,59; IC 95%: 0,49-0,72; p < 0,0001). L’analisi dei sottogruppi ha, inoltre, evidenziato un importante effetto sinergico della combinazione nelle pazienti con mutazione BRCA e deficit della ricombinazione omologa (HRD) con una PFS media di 37,2 mesi ed un hazard ratio di 0,31 e 0,33, rispettivamente. L’aggiunta di olaparib a bevacizumab, invece, ha mostrato un effetto quasi nullo nel sottogruppo HRD-negativo o con stato sconosciuto dell’HRD, dove la PFS mediana è risultata pari a 16,9 mesi nel braccio sperimentale versus 16 mesi nel braccio di controllo.
Questi dati supportano l’ipotesi dell’effetto sinergico di olaparib e bevacizumab nelle pazienti BRCA mutate e con HRD sottolineando l’importanza dello stato di HRD quale nuovo fattore prognostico e predittivo di risposta agli inibitori di PARP e sollevando, contestualmente, le criticità legate ai costi e alla affidabilità e riproducibilità del test per HRD non ancora standardizzato.
Attualmente la combinazione dei due farmaci non è ancora autorizzata in Italia e l’utilizzo dell’olaparib come terapia di mantenimento nelle pazienti in risposta ad una chemioterapia di prima linea a base di platino resta limitato alle sole pazienti con mutazione di BRCA.
Overall Survival with Osimertinib in Untreated, EGFR-Mutated Advanced NSCLC

I dati dello studio FLAURA pubblicati nel 2018 hanno rivoluzionato lo scenario terapeutico di prima linea dei pazienti affetti da NSCLC tanto che le linee guida internazionali e nazionali suggeriscono l’utilizzo di osimertinib, inibitore tirosin-chinasico (TKI) di terza generazione, come farmaco di scelta nei pazienti naïve con mutazioni attivanti classiche del gene EGFR (Ex19dels, L858R). Lo studio infatti aveva mostrato una maggiore efficacia di osimertinib rispetto ai TKI di prima generazione (erlotinib e gefitinib) con un vantaggio statisticamente significativo nel primary end-point: sopravvivenza libera da progressione (PSF) mediana 18,9 versus 10,8 mesi (HR 0,46; IC 95%: 0,37–0,57; p < 0,001). Il vantaggio in PFS era stato osservato in tutti i sottogruppi predefiniti, incluso quello dei pazienti con metastasi cerebrali. Al momento del data cut-off per l’analisi della PFS, i dati di sopravvivenza globale (OS) non erano ancora maturi e la mediana non era stata raggiunta nei due bracci di trattamento.
Nel presente articolo Ramalingam et al. hanno riportato i dati dell’analisi finale della OS dello studio confermando il vantaggio del TKI EGFR di terza generazione. La OS mediana, infatti, è stata pari a 38,6 mesi nel gruppo trattato con osimertinib e 31,8 mesi nel gruppo di controllo (HR 0,799; IC 95%: 0,641–0,997; p = 0,0462), con un profilo di sicurezza simile tra i due bracci di trattamento. Il beneficio con osimertinib rispetto al braccio di controllo è presente nella maggior parte dei sottogruppi.
Considerato il beneficio clinico di osimertinib in prima linea restano da definire le migliori terapie sequenziali basandosi anche sui profili di resistenza molecolare. Nello studio FLAURA il 68% dei pazienti trattati in prima linea con osimertinib ha ricevuto al momento della progressione un trattamento chemioterapico e il 29% un altro TKI, mentre il 47% dei pazienti inclusi nel braccio controllo ha ricevuto osimertinib in seguito a riscontro della mutazione T790M di EGFR.
Dati preliminari suggeriscono che i meccanismi di resistenza di osimertinib in prima linea sono simili a quelli osservati in pazienti con mutazione T790M che ricevono osimertinib come terapia di seconda linea. Tali meccanismi di resistenza sono analoghi a quelli associati a mutazioni meno frequenti che si riscontrano in pazienti che hanno resistenza a TKI EGFR di prima e seconda generazione. Lo studio ELIOS ci fornirà rilevanti acquisizioni su tali meccanismi di resistenza così come gli studi ORCHARD e SAVANNAH ci daranno importanti indicazioni sul trattamento più efficace dopo progressione di malattia ad osimertinib.

Nell’era della target therapy e dell’immunoterapia, la chemioterapia a base di platino resta una importante opzione terapeutica in prima linea nei pazienti on NSCLC non oncogene-addicted.
Lo studio COMPASS è stato condotto con l’obiettivo di valutare l’efficacia e sicurezza di bevacizumab con o senza pemetrexed come terapia di mantenimento dopo terapia di induzione con carboplatino, pemetrexed e bevacizumab. In questo studio, la sopravvivenza libera da progressione (PFS) è risultata significativamente migliore con pemetrexed più bevacizumab rispetto a bevacizumab in monoterapia (HR 0,67; IC 95%: 0,57-0,79; p = 0,001), senza un incremento significativo della sopravvivenza. Tuttavia, gli Autori hanno sottolineato che nello studio in maniera inaspettata la sopravvivenza globale (OS) è stata superiore di 7 mesi in entrambi i gruppi di trattamento rispetto ai dati attesi, restando superiore anche dopo aver escluso i pazienti con mutazioni sensibilizzanti di EGFR a prognosi migliore. Una spiegazione della più lunga sopravvivenza nello studio COMPASS può essere ricercata nel notevole progresso delle terapie sistemiche per i pazienti con NSCLC metastatico, in particolare nell’utilizzo degli inibitori dei checkpoint immunitari (ICI).
Considerando i dati di efficacia degli ICI in associazione a chemioterapia in prima linea e la recente approvazione del pembrolizumab in associazione a pemetrexed e chemioterapia contenente platino, il ruolo della terapia di mantenimento con pemetrexed e bevacizumab dopo chemioterapia di induzione nei pazienti con NSCLC non squamoso potrebbe essere limitato a sottogruppi di pazienti, ad esempio quelli con controindicazione all’utilizzo di ICI. Un risvolto terapeutico futuro potrebbe derivare dai dati dello studio giapponese APPLE in corso che sta valutando l’efficacia del regime COMPASS in associazione ad atezolizumab.
