Lorlatinib in advanced ROS1-positive non-small-cell lung cancer: a multicentre, open-label, single-arm, phase 1-2 trial

I riarrangiamenti cromosomici del gene che codifica la tirosin-chinasi del proto-oncogene ROS1 (ROS1) sono documentabili in circa l’1%-2% dei pazienti con diagnosi di tumore polmonare non a piccole cellule (NSCLC), di cui costituiscono un sottogruppo molecolare distinto suscettibile della inibizione terapeutica mediante specifici inibitori chinasici. Nonostante l’inibitore multichinasico di prima generazione crizotinib produca risposte obiettive in circa il 70% dei pazienti ROS1-positivi, con una sopravvivenza mediana libera da progressione di circa 19 mesi ed una sopravvivenza globale di 50 mesi, invariabilmente in tutti i pazienti si instaura una resistenza al trattamento.
Il lorlatinib è un nuovo potente inibitore tirosin-chinasico di terza generazione, dotato di spiccata attività a livello intracranico, ed efficace contro le più comuni mutazioni di resistenza al crizotinib.
In questo studio multicentrico di fase 1-2, a singolo braccio, 69 pazienti affetti da NSCLC ROS1-positivo hanno ricevuto lorlatinib alla dose di 100 mg al giorno (dose-escalation: 10-100 mg nella sola fase 1). Nei pazienti pretrattati con crizotinib, il tasso di risposta è stato del 35% (IC 95%: 21-52), mentre nei pazienti non pretrattati con inibitori chinasici (TKI) il tasso di risposta è stato del 62% (IC 95%: 38-82) con il 10% di risposte complete confermate. Ad un follow-up mediano di 21 mesi, la sopravvivenza mediana libera da progressione è stata di 8,5 mesi (IC 95%: 4,7-15,2) e di 21,0 mesi (IC 95%: 4,2-31,9) nei pazienti pretrattati con crizotinib e TKI-naïve, rispettivamente. Va infine sottolineato come il 64% (IC 95%: 31-89) dei pazienti TKI-naïve ed il 50% (IC 95%: 29-71) dei pazienti pretrattati con crizotinib abbiano avuto una risposta intracranica confermata.
I risultati di questo studio dimostrano come il lorlatinib sia efficace nei pazienti pretrattati con TKI, e come l`utilizzo in prima linea possa rappresentare un’opzione terapeutica superiore ai farmaci di prima generazione, potenzialmente rallentando lo sviluppo di resistenza sia farmacocinetica che farmacodinamica, con un soddisfacente profilo di tossicità.

I pazienti con diagnosi di cancro hanno un rischio incrementato di sviluppare eventi tromboembolici venosi (VTE) rispetto alla popolazione generale. In particolare, i pazienti affetti da carcinoma polmonare non a piccole cellule (NSCLC) hanno un rischio di circa 8%-15% di sviluppare almeno un evento tromboembolico durante il corso della malattia. Tuttavia il rischio di sviluppare VTE sembra essere variabile a seconda non solo dell’istologia ma anche di distinte caratteristiche biomolecolari specifiche del NSCLC, incluso lo stato mutazionale di noti driver oncogenici.
In questa analisi retrospettiva dello studio METROS, uno studio di fase 2 che ha valutato l`efficacia del crizotinib nei pazienti affetti da NSCLC ROS1-positivo e MET deregolato, l’incidenza di VTE nella coorte ROS1-riarrangiata è risultata del 41,6% (20/48), significativamente più alta rispetto alla coorte MET-deregolata, seguita prospetticamente nello stesso intervallo di tempo, in cui l’incidenza è risultata del 15,3% (p = 0,03), quest’ultima in linea con quanto noto dalla letteratura. Inoltre, circa il 35% (7/20) dei pazienti che hanno sviluppato VTE ha avuto eventi tromboembolici venosi multipli. In questo studio è stato inoltre osservato come la maggior parte degli eventi VTE si sia sviluppata al momento della diagnosi (32,1%) o della progressione radiologica di malattia (35,7%), a suggerire un nesso causale tra attività di malattia e rischio di sviluppare VTE. Il tempo mediano dalla diagnosi al primo evento tromboembolico è stato 81,5 giorni (IQR: 12,2-250). Nonostante i meccanismi molecolari responsabili di questa associazione non siano stati ancora elucidati, è possibile che la mucina prodotta dalle cellule tumorali ROS1-positive possa legare la L-selectina, attivando in questo modo l’adesione ed aggregazione piastrinica e conseguentemente generare trombosi parietale ed embolizzazione.
I risultati di questa analisi dimostrano come i pazienti affetti da NSCLC ROS1-positivo abbiano un rischio 5 volte superiore di sviluppare VTE rispetto alla popolazione generale di pazienti diagnosticati con NSCLC, e sottolineano la necessità di includere le caratteristiche biomolecolari e genomiche tumorali negli algoritmi di diagnosi, profilassi e trattamento degli eventi tromboembolici venosi.

Un elevato indice di massa corporea (BMI) è stato recentemente correlato con un’incrementata efficacia dell’immunoterapia in pazienti affetti da melanoma. Anche se il meccanismo sottostante questa associazione non è stato ancora completamente elucidato, è possibile che l’obesità induca un basso grado di infiammazione sistemica e di alterata risposta immunitaria. Inoltre, l’obesità è in grado di rendere i linfociti T, PD-1 positivi, disfunzionali all`interno del microambiente tumorale tramite la produzione di leptina, legando in questo modo un elevato BMI ad una migliore risposta antitumorale indotta dagli inibitori del checkpoint immunitario.
In questa pooled analisis di dati individuali da 4 studi clinici multicentrici (BIRCH, FIR, OAK, POPLAR) e che ha incluso 2.110 pazienti, Kichenadassee et al. hanno dimostrato come un BMI ≥ 30 sia associato ad outcomes clinici significativamente migliori nei pazienti con carcinoma del polmone non a piccole cellule (NSCLC) trattati con l’inibitore del PD-L1 atezolizumab.
In particolare, i pazienti obesi (BMI ≥ 30 ) e sovrappeso (BMI = 26-29) hanno ottenuto una riduzione del rischio di morte del 36% (HR 64; IC 95%: 0,51-0,81]) e del 19% (HR 81; IC 95%: 0,68-0,95]) rispetto ai pazienti normopeso trattati con atezolizumab. Questa associazione è rimasta significativa anche dopo aver aggiustato gli outcomes di sopravvivenza per variabili potenzialmente confondenti in un modello di analisi multivariata (HR 0.69; IC 95%: 0,54-0,87; riduzione del rischio di morte del 31% per i pazienti obesi rispetto ai pazienti normopeso). La stessa associazione tra elevato BMI ed efficacia clinica dell’atezolizumab è stata inoltre osservata nel sottogruppo di pazienti con un livello di espressione del PD-L1 sul tessuto tumorale del 50% o maggiore. Diversamente, gli autori non hanno riscontrato alcuna associazione tra BMI e sopravvivenza nei pazienti trattati con docetaxel.
In conclusione, seppur limitato dalla presenza di diversi bias di selezione, in questo studio un elevato BMI è stato associato con una sopravvivenza significativamente superiore nei pazienti affetti da NSCLC trattati con atezolizumab. Questi dati suggeriscono che il valore basale di BMI dovrebbe essere incluso come fattore di stratificazione negli studi clinici che valutano inibitori del checkpoint immunitario.
