Il carcinoma mammario è la seconda causa di morte per cancro nelle donne, ma la diagnosi precoce e un trattamento appropriato possono migliorare considerevolmente i risultati. Di conseguenza, molte nazioni industrializzate hanno implementato programmi di screening mammografico su larga scala. Le principali organizzazioni mediche e governative raccomandano lo screening per tutte le donne di età compresa tra 40 e 50 anni. Negli Stati Uniti e nel Regno Unito, complessivamente, ogni anno vengono eseguiti oltre 42 milioni di esami. Nonostante l’adozione diffusa della mammografia, l’interpretazione rimane impegnativa essendo gravata da tassi elevati di falsi positivi e falsi negativi. La precisione raggiunta dagli esperti nella rilevazione del cancro varia ampiamente.
I tumori che sfuggono ad un programma di screening vengono potenzialmente detettati in uno stadio più avanzato e meno suscettibile di trattamento.
Nell’articolo, McKinney et al. presentano un sistema ad intelligenza artificiale (AI) in grado di predire la malattia. Per valutare le sue prestazioni in ambito clinico, gli autori hanno creato un ampio set di dati rappresentativo di popolazione nel Regno Unito e negli Stati Uniti. Dall’analisi dei dati si dimostra una riduzione assoluta del 5,7% e 1,2% (USA e Regno Unito) in termini di falsi positivi e 9,4% e 2,7% rispettivamente di falsi negativi. In uno studio indipendente condotto da sei radiologi, il sistema di intelligenza artificiale ha sovraperformato tutti i “lettori umani”. È stata eseguita una simulazione in cui il sistema di intelligenza artificiale ha partecipato al processo di doppia lettura utilizzato nel Regno Unito e si è dimostrato che il sistema AI ha mantenuto prestazioni non inferiori e ha ridotto il carico di lavoro di secondo lettore dell’88%. Questa solida valutazione del sistema di intelligenza artificiale apre la strada a studi clinici per migliorare l’accuratezza e l’efficacia dello screening del carcinoma mammario.

Negli ultimi tempi si sta assistendo ad un interesse sempre maggiore del ruolo della nutrizione ed in particolare della restrizione calorica nello sviluppo e nella progressione dei tumori maligni. Studi preclinici su larga scala forniscono forti evidenze che dimostrano che il digiuno intermittente sia in grado di minimizzare i processi anabolici di sintesi proteica, di crescita e riproduzione cellulare con l’effetto conseguente di favorire i meccanismi di riparo, di incrementare la resistenza allo stress ossidativo e di favorire la sopravvivenza cellulare grazie allo shifting metabolico verso la chetogenesi e alla riduzione dei processi infiammatori. Partendo da questi dati, arrivare ad affermare che il digiuno intermittente possa supportare l’ipotesi di un impatto favorevole sullo stato di salute e sulla resistenza alle malattie nell’uomo, è stato un passaggio pressocché diretto e molti studi sono stati condotti sull’animale e sull’uomo sull’uso di questa pratica con l’obiettivo di migliorare lo stato di salute, di prevenire alcune patologie e di rallentare i processi di invecchiamento.
Nonostante gli studi preclinici abbiano dimostrato un robusto effetto del digiuno intermittente sull’andamento di un ampio range di disordini metabolici cronici che includono obesità, diabete, malattie cardiovascolari, tumori e malattie cerebrali neurodegenerative, le evidenze di tali effetti sull’uomo sono ancora oggetto di discussione. In particolare, negli studi sull’uomo i tre regimi di digiuno intermittente più studiati sono il digiuno a giorni alterni, il digiuno di due giorni su sette e l’alimentazione con restrizione calorica quotidiana; in questi studi i benefici osservati riguardavano il controllo della glicemia, della pressione arteriosa, della frequenza cardiaca e la riduzione del grasso addominale.
Recentemente, studi di restrizione calorica e digiuno intermittente sono stati focalizzati sull’invecchiamento e sull’aspettativa di vita. Tuttavia, in due studi recenti condotti su primati, i risultati della restrizione calorica sono stati concordi rispetto al miglioramento della salute globale dei casi trattati, ma discordanti sull’impatto sulla sopravvivenza. Mentre studi clinici hanno evidenziato un miglioramento delle capacità cognitive in termini di memoria verbale, funzioni esecutive, capacità globale cognitiva, oltre che un miglioramento delle principali funzioni cardiovascolari e relativi marcatori, per quanto riguarda i pazienti affetti da tumore, studi clinici sono ancora in corso sull’effetto sugli indicatori di progressione neoplastica e sul controllo degli effetti collaterali da chemioterapia, ma nessuno studio è stato ancora pubblicato sull’efficacia del digiuno intermittente sulla storia naturale delle neoplasie.
La difficoltà nel condurre studi clinici controllati sull’uomo sull’efficacia della restrizione calorica o digiuno intermittente sta soprattutto nella scelta dei migliori regimi e nella compliance agli stessi da parte dei pazienti. Infine, l’ipotesi di utilizzare farmaci capaci di mimare gli effetti biologici del digiuno sull’uomo (metformina, nicotinamide riboside o inibitori di mTOR) rimane da essere validata e comunque è ragionevole pensare che gli eventuali effetti metabolici possano non essere del tutto sovrapponibili a quelli degli interventi alimentari.

Un elevato indice di massa corporea (BMI) è stato recentemente correlato con un’incrementata efficacia dell’immunoterapia in pazienti affetti da melanoma. Anche se il meccanismo sottostante questa associazione non è stato ancora completamente elucidato, è possibile che l’obesità induca un basso grado di infiammazione sistemica e di alterata risposta immunitaria. Inoltre, l’obesità è in grado di rendere i linfociti T, PD-1 positivi, disfunzionali all`interno del microambiente tumorale tramite la produzione di leptina, legando in questo modo un elevato BMI ad una migliore risposta antitumorale indotta dagli inibitori del checkpoint immunitario.
In questa pooled analisis di dati individuali da 4 studi clinici multicentrici (BIRCH, FIR, OAK, POPLAR) e che ha incluso 2.110 pazienti, Kichenadassee et al. hanno dimostrato come un BMI ≥ 30 sia associato ad outcomes clinici significativamente migliori nei pazienti con carcinoma del polmone non a piccole cellule (NSCLC) trattati con l’inibitore del PD-L1 atezolizumab.
In particolare, i pazienti obesi (BMI ≥ 30 ) e sovrappeso (BMI = 26-29) hanno ottenuto una riduzione del rischio di morte del 36% (HR 64; IC 95%: 0,51-0,81]) e del 19% (HR 81; IC 95%: 0,68-0,95]) rispetto ai pazienti normopeso trattati con atezolizumab. Questa associazione è rimasta significativa anche dopo aver aggiustato gli outcomes di sopravvivenza per variabili potenzialmente confondenti in un modello di analisi multivariata (HR 0.69; IC 95%: 0,54-0,87; riduzione del rischio di morte del 31% per i pazienti obesi rispetto ai pazienti normopeso). La stessa associazione tra elevato BMI ed efficacia clinica dell’atezolizumab è stata inoltre osservata nel sottogruppo di pazienti con un livello di espressione del PD-L1 sul tessuto tumorale del 50% o maggiore. Diversamente, gli autori non hanno riscontrato alcuna associazione tra BMI e sopravvivenza nei pazienti trattati con docetaxel.
In conclusione, seppur limitato dalla presenza di diversi bias di selezione, in questo studio un elevato BMI è stato associato con una sopravvivenza significativamente superiore nei pazienti affetti da NSCLC trattati con atezolizumab. Questi dati suggeriscono che il valore basale di BMI dovrebbe essere incluso come fattore di stratificazione negli studi clinici che valutano inibitori del checkpoint immunitario.
