Lo studio MEDIOLA è un basket trial multicentrico, di fase I/II, volto a indagare efficacia e sicurezza di un PARP inibitore (olaparib) in associazione con durvalumab (un anticorpo monoclonale inibitore di PD-L1), nelle pazienti affette da neoplasia mammaria (breast cancer, BC) avanzata con mutazione germinale di BRCA1 e/o BRCA2. Lo studio prevedeva 4 coorti iniziali che comprendevano, oltre a quella già citata, pazienti con neoplasia ovarica avanzata e mutazione germinale di BRCA, pazienti con neoplasia gastrica avanzata e pazienti con neoplasia polmonare a piccole cellule in progressione di malattia.
Per quanto riguarda le pazienti con BC, si trattava di pazienti HER2-negative, che non dovevano aver ricevuto più di due linee di chemioterapia per malattia metastatica. Le pazienti hanno ricevuto olaparib 300 mg 2 volte al giorno per os per 4 settimane e, in seguito, olaparib (allo stesso dosaggio) associato a durvalumab 1,5 g per via endovenosa ogni 4 settimane, fino a progressione di malattia.
Gli endpoints primari erano la sicurezza, la tollerabilità e il controllo di malattia a 12 settimane.
Tra giugno 2016 e maggio 2017 sono state arruolate 34 pazienti; 11 di queste (32%) hanno riportato eventi avversi almeno di grado 3, i più frequenti sono stati: anemia (12%), neutropenia (9%) e pancreatite (6%). Tre pazienti (9%) hanno interrotto il trattamento a causa degli eventi avversi. Non si sono verificati decessi correlati al trattamento. La combinazione di olaparib e durvalumab ha mostrato un’incoraggiante attività tumorale, con 24 pazienti (su 30 eleggibili per l’analisi) che hanno avuto un controllo di malattia a 12 settimane. Inoltre, il profilo di sicurezza è simile a quello osservato per gli studi dove i due farmaci erano utilizzati in monoterapia.
Si tratta del primo studio a riportare risultati di questo tipo in pazienti con BC e recettori ormonali positivi, purtroppo tuttavia non sono riportati dati sulle precedenti linee ormonali ricevute nel setting metastatico né riguardo ad una eventuale precedente associazione con i farmaci inibitori delle cicline, mentre ormai sappiamo come le precedenti linee di trattamento possano modificare il complesso panorama immunitario. Ulteriori studi saranno necessari per individuare fattori predittivi di risposta a tali trattamenti e se l’associazione con durvalumab sia in grado di migliorare gli outcomes a lungo termine, rispetto ad olaparib in monoterapia.

Desmedt e colleghi hanno analizzato retrospettivamente i dati delle pazienti che avevano partecipato allo studio adiuvante BIG 2-98, che comprendeva 2.887 pazienti, mettendo a confronto i regimi chemioterapici contenenti o meno docetaxel.
Scopo dell’analisi era valutare se l’efficacia terapeutica dei regimi docetaxel-based poteva differire da quella non-docetaxel-based nelle pazienti con carcinoma mammario (breast cancer, BC) sulla base del loro indice di massa corporea (body mass index, BMI). È noto infatti come la farmacocinetica e la farmacodinamica dei farmaci lipofili dipendano dalla composizione corporea, pertanto i taxani, farmaci lipofili, hanno un maggior volume di distribuzione laddove il tessuto adiposo è più rappresentato. Le pazienti sono state suddivise in 3 categorie sulla base del loro BMI (kg/m2): normopeso se questo andava da 18,5 fino a 25, sovrappeso le pazienti con BMI da 25 fino a 30, obese quelle con BMI superiore o uguale a 30. L’endpoint primario era la disease-free survival (DFS) mentre l’overall survival (OS) costituiva l’endpoint secondario.
Si è osservato una riduzione di DFS e OS all’aumentare del BMI nelle pazienti trattate con regimi docetaxel-based. L’hazard ratio per DFS e OS era di 1,12 (IC 95%: 0,98 – 1,50; p = 0,21) e 1,27 (IC 95%: 1,01 – 1,60; p = 0,04), rispettivamente, per le pazienti sovrappeso rispetto alle pazienti magre, ed era di 1,32 (IC 95%: 1,08 – 1,62; p = 0,007) e 1,63 (IC 95%: 1,27 – 2,09; p < 0,001), rispettivamente, per le pazienti obese rispetto alle pazienti magre. Risultati analoghi sono stati ottenuti considerando separatamente le neoplasia positive per recettori estrogenici (ER-positive) e quelle ER-negative.
Questa analisi retrospettiva mette in luce come la risposta ad un trattamento comprendente docetaxel risenta della composizione corporea e, quindi, del BMI e questo può essere utilizzato per riconsiderare il rapporto rischio-beneficio nell’uso dei taxani nel BC. È vero però che, oggigiorno, il BMI non è l’unico né il più completo modo per valutare la composizione corporea. La dual X-ray absorptiometry (DEXA) rappresenta il gold-standard, anche se il suo utilizzo nella pratica clinica è limitato. Altri metodi più accessibili sono l’analisi della composizione corporea basata sulla TC e la bioimpedenzometria (BIA) che valuta la composizione corporea sulla base dell’impedenza, che aumenta all’aumentare della massa grassa e si riduce all’aumentare della massa magra. In ogni caso, studi prospettici saranno necessari per confermare questi dati e modificare la pratica clinica.
