Il ruolo della terapia di mantenimento per il cancro della giunzione gastrica (GC) o gastroesofageo (GEJC) è ad oggi dibattuto. Lo studio di fase III JAVELIN Gastric 100 ha valutato proprio il ruolo del mantenimento dell’anti-PD-L1 avelumab dopo la chemioterapia di induzione di prima linea per GC / GEJC.
I pazienti eleggibili avevano GC o GEJC non resecabile, HER-2 negativo, localmente avanzato o metastatico. I pazienti senza malattia progressiva dopo 12 settimane di chemioterapia di prima linea con oxaliplatino più una fluoropirimidina sono stati assegnati in modo casuale 1: 1 ad avelumab 10 mg / kg ogni 2 settimane o hanno continuato la chemioterapia, stratificati per regione (Asia vs non Asia). L’endpoint primario era la sopravvivenza globale (OS) dopo la chemioterapia di induzione in tutti i pazienti assegnati in modo casuale o nella popolazione PD-L1-positiva assegnata in modo casuale (≥ 1% delle cellule tumorali).
Un totale di 805 pazienti ha ricevuto la terapia di induzione; 499 sono stati assegnati in modo casuale ad avelumab (n = 249) o alla chemioterapia continuata (n = 250). La OS mediana è stata di 10,4 mesi (IC 95%: 9,1 – 12,0) rispetto a 10,9 mesi (IC 95%: 9,6 – 12,4) e la percentuale di OS a 24 mesi è stata del 22,1% contro 15,5% rispettivamente con avelumab rispetto alla chemioterapia (HR 0,91; IC 95%: 0,74 – 1,11; p = 0,1779). Nella popolazione PD-L1-positiva (n = 54), l’HR per OS era 1,13 (IC 95%: 0,57 – 2,23; p = 0,6352). In un’analisi esplorativa della popolazione PD-L1 positiva, definita come punteggio positivo combinato ≥ 1 (dosaggio 22C3; n = 137), l’OS mediana è stata di 14,9 mesi (IC 95%: 8,7 – 17,3) con avelumab rispetto a 11,6 mesi (IC 95%: 8,4 – 12,6) con chemioterapia (HR non stratificato 0,72; IC 95%: 0,49 – 1,05). Gli eventi avversi correlati al trattamento si sono verificati in 149 (61,3%) e 184 (77,3%) pazienti, inclusi casi di grado ≥ 3 in 31 (12,8%) e 78 (32,8%) pazienti, rispettivamente.
Pertanto, sulla base di questi dati, il mantenimento con avelumab non ha dimostrato una OS superiore rispetto alla prosecuzione della chemioterapia nei pazienti con GC avanzato o GEJC né in generale né in una popolazione pre-specificata positiva per PD-L1.

I riarrangiamenti genici del proto-oncogene ROS1 rappresentano driver oncogenici nel carcinoma polmonare non a piccole cellule (NSCLC).
Crizotinib è da tempo approvato come terapia di prima linea per il NSCLC positivo alla fusione ROS1 metastatico. Tuttavia, crizotinib ha una scarsa penetrazione nel SNC ed è attivamente portato via dal SNC tramite la glicoproteina P. La piccola molecola TKI (tyrosine kinase inhibitor) entrectinib è un potente inibitore di ROS1, specificamente progettato per attraversare la barriera emato-encefalica e rimanere attivo all’interno del SNC. Contrariamente a crizotinib, i modelli preclinici hanno confermato che entrectinib è un debole substrato della glicoproteina P e quindi raggiunge concentrazioni elevate nel SNC, associate a una forte efficacia nei modelli di tumore a livello cerebrale. Sulla base di questi risultati positivi, entrectinib è stato approvato dalla Food and Drug Administration statunitense e dall’EMA per i pazienti con NSCLC positivo alla fusione ROS1.
Sono stati recentemente pubblicati i dati aggiornati di efficacia e sicurezza per entrectinib in pazienti con NSCLC positivo alla fusione ROS1, sulla base di un’analisi integrata di ALKA-372-001, STARTRK-1 e STARTRK-2 con più pazienti e follow-up più lungo.
Si trattava di soggetti con malattia localmente avanzata o metastatica con o senza metastasi del SNC che hanno ricevuto entrectinib ≥ 600 mg per via orale una volta al giorno. Gli endpoint co-primari erano il tasso di risposta obiettiva (ORR), valutato da una revisione centrale indipendente in cieco, e la durata della risposta (DoR). Gli endpoint secondari includevano la sopravvivenza libera da progressione (PFS), sopravvivenza globale (OS), ORR intracranica, DoR intracranica, PFS intracranica e safety.
In totale, sono stati valutati 161 pazienti con un follow-up di ≥ 6 mesi. La durata mediana del trattamento è stata di 10,7 mesi. L’ORR è risultata pari al 67,1% (n = 108; IC 95%: 59,3 – 74,3) e le risposte sono state durevoli (tasso di DoR a 12 mesi 63%; DoR mediano 15,7 mesi). La percentuale di PFS a 12 mesi è risultata pari al 55% (PFS mediana 15,7 mesi) e la percentuale di OS a 12 mesi all’81% (OS mediana non stimabile). In 24 pazienti con metastasi del SNC al basale, misurabili mediante revisione centrale indipendente in cieco, l’ORR intracranico è stato del 79,2% (n = 19; IC 95%: 57,9 – 92,9), la PFS intracranica mediana di 12,0 mesi (IC 95%: 6,2 – 19,3) e la DoR intracranica mediana di 12,9 mesi. Il profilo di sicurezza in questa analisi aggiornata era simile a quello riportato nell’analisi primaria.
Gli autori del lavoro hanno pertanto concluso che entrectinib ha continuato a dimostrare un elevato livello di beneficio clinico per i pazienti con NSCLC positivo alla fusione ROS1, inclusi i pazienti con metastasi del SNC.
