La rivoluzione dell’immunoterapia ha cambiato il paradigma di trattamento nel tumore del polmone non a piccole cellule (NSCLC) in stadio avanzato. Lo studio IMpower010 ha voluto valutare l’anticipazione dell’immunoterapia nel setting adiuvante.
In particolare, questo studio di fase III, multicentrico, randomizzato, ha arruolato 1.280 pazienti, che sono stati sottoposti a trattamento chemioterapico adiuvante a base di cisplatino dopo resezione chirurgica completa per NSCLC dallo stadio IB allo stadio IIIA. Al completamento del trattamento chemioterapico i pazienti che ancora soddisfacevano i criteri di ammissibilità (n = 1.005) sono stati randomizzati con un rapporto 1:1 a ricevere atezolizumab al dosaggio di 1.200 mg ogni 21 giorni (per un massimo di 16 cicli) oppure la migliore terapia di supporto.
Il disegno dello studio prevedeva che la sopravvivenza libera da malattia (DFS) (endpoint primario) e la sopravvivenza globale (OS) (endpoint secondario) fossero valutati in 3 popolazioni: la DFS nel sottogruppo con malattia in stadio II-IIIA e con espressione di PD-L1 ≥ 1% sulle cellule tumorali, successivamente la DFS in tutti i pazienti randomizzati con malattia in stadio II-IIIA, in seguito la DFS nella popolazione Intent-To-Treat (ITT) (stadio IB-IIIA) ed, infine, l’OS nella popolazione ITT.
Atezolizumab ha mostrato un vantaggio statisticamente significativo in termini di DFS rispetto al braccio di controllo nella popolazione di pazienti in stadio II-IIIA con espressione di PD-L1 ≥ 1%; in questo sottogruppo, infatti, la DFS mediana non è ancora stata raggiunta nel braccio trattato con atezolizumab, mentre è risultata di 35,3 mesi nel braccio trattato con la migliore terapia di supporto, con una riduzione del 34% del rischio di recidiva della malattia o decesso nei pazienti trattati con l’anti-PD-L1 (HR 0,66; IC 95%: 0,50-0,98; p = 0,0039). Nell’intera coorte di pazienti in stadio II-IIIA, atezolizumab ha mostrato una riduzione del 21% del rischio di recidiva della malattia o decesso rispetto al trattamento con la migliore terapia di supporto , con una DFS mediana pari, rispettivamente, a 42,3 mesi versus 35,3 mesi (HR 0,79; IC 95%: 0,64-0,96; p = 0,0205). Nella analisi ad interim, la differenza di DFS fra i due bracci di trattamento non ha raggiunto la significatività statistica nella popolazione ITT (HR 0,81; IC 95%: 0,67-0,99; p = 0,0395).
Gli eventi avversi, come atteso, sono stati più frequenti nei pazienti trattati con l’anti-PD-L1 con una percentuale di tutti i gradi pari al 92% nel braccio di trattamento attivo versus il 70,7% nel braccio di confronto; eventi avversi di grado 3/4 hanno avuto un’incidenza rispettivamente del 21,8% e 11,5%. Inoltre, il 18,2% dei pazienti trattati con il farmaco immunoterapico ha sviluppato eventi avversi che hanno richiesto l’interruzione del trattamento.
Queste risultati positivi, insieme a un profilo di sicurezza coerente con quello noto, suggeriscono che atezolizumab dopo chemioterapia adiuvante potrebbe rappresentare un’opzione di trattamento promettente nei pazienti con NSCLC resecato in stadio II-IIIA con una espressione del PD-L1 > 1%.
