Come per altri tumori solidi, anche nel  carcinoma della cervice uterina metastatico sono sempre di più le prove di efficacia dell’immunoterapia.
Diversi studi sono stati condotti a riguardo, sia con immune checkpoint inhibitors (ICIs) in monoterapia (dei quali il più promettente è l’anti-PD-1 cemiplimab), sia in terapie di combinazione con agenti citotossici standard. L’ultimo di questa tipologia di trial è il KEYNOTE-826, presentato all’ESMO di quest’anno dalla prof.ssa Nicoletta Colombo e recentemente pubblicato sul New England Journal of Medicine.
L’obiettivo dello studio è stato quello di valutare l’efficacia in termini di progression-free survival (PFS) e di overall survival (OS) dell’aggiunta di pembrolizumab al trattamento chemioterapico standard a base di platino e bevacizumab in donne con carcinoma della cervice persistente, recidivato o metastatico all’esordio.
Lo studio, in doppio cieco, di fase III, con randomizzazione 1:1 ha coinvolto 151 centri in 19 nazioni e ha arruolato un totale di 617 pazienti, con caratteristiche ben bilanciate nei due bracci di trattamento.
Con un follow-up mediano di 22,0 mesi, si è osservata una OS nettamente maggiore nel braccio sperimentale rispetto al braccio standard sia nella popolazione generale sia nei diversi sottogruppi (classificati in base al combined-positive score, CPS). In particolare, per quanto riguarda il sottogruppo con CPS > 1, la stima delle pazienti vive a 24 mesi è stata del 53% nel braccio sperimentale e del 41,7% nel braccio di controllo (hazard ratio per morte 0,64; IC 95%: 0,50 – 0,81; p < 0,001); nella popolazione intention-to-treat è stata del 50,4% nella terapia con ICI vs 40,4% nel braccio standard.
La OS mediana non è stata raggiunta nel braccio sperimentale, mentre nel braccio con placebo è stata stimata essere di 16,3 mesi. Lo studio ha dimostrato inoltre un beneficio in termini di overall response rate e time-to-deterioration con un importante miglioramento della qualità della vita.
Alla luce di questi ed altri risultati presentati nel lavoro, la combinazione di pembrolizumab e chemioterapia a base di platino + bevacizumab dovrebbe essere considerata il nuovo standard di I linea nelle donne con carcinoma della cervice uterina.

I depositi tumorali (tumor deposits, TD) sono noduli neoplastici di qualsiasi forma e dimensione all’interno dell’area del drenaggio linfatico del tumore primitivo. Secondo la classificazione TNM – AJCC 2017 attualmente in uso, una neoplasia colica con “evidenza di depositi tumorali satelliti nella sottosierosa o nei tessuti non peritonealizzati pericolici o perirettali senza metastasi linfonodali regionali” viene considerata come una neoplasia con localizzazioni linfonodali (N1) e pertanto uno stadio III di malattia.
Il trattamento adiuvante del carcinoma del colon in stadio III è stato ampliamente studiato negli ultimi decenni e ha raggiunto il suo vertice nella collaborazione internazionale del gruppo IDEA. Nonostante i risultati non siano di semplice interpretazione, è possibile concludere che 3 mesi di CAPOX possono essere proposti nelle categorie a rischio minore.
Da qui nasce l’esigenza di una sempre migliore stadiazione e stratificazione del rischio dei pazienti.
In questo articolo, recentemente pubblicato su Annals of Oncology, è stato valutato il valore prognostico dei TD e l’impatto della loro aggiunta alla conta dei linfonodi metastatici nel carcinoma del colon in stadio III.
L’analisi post-hoc ha valutato 2.080 soggetti arruolati all’interno del trial di fase III CALGB/SWOG 80702. Di questi, 524 (26%) sono stati classificati come TD-positivi e 1.504 (74%) TD-negativi.
Tra i TD-positivi, sono stati osservati 80 (15,4%) pazienti senza metastasi linfonodali (N1c secondo TNM – AJCC 2917), 239 (46,1%) pazienti pN1a/b e 200 (38,55%) pN2.
Nella popolazione generale, la presenza di TD ha dimostrato avere un impatto negativo tanto sulla DFS (adjusted hazard ratio aHR 1,59; IC 95%: 1,28 – 1,91) quanto sulla OS (aHR 1,52; IC 95%: 1,18 – 1,95). Inoltre nei soggetti TD-positivi, il numero dei TD ha un effetto negativo lineare sulla DFS e OS. L’aggiunta dei TD al numero di linfonodi metastatici permette anche di ristadiare i pazienti dallo stadio pN1 allo stadio pN2, con un impatto significativo sulla sopravvivenza.
Questi e altri dati presentati nello studio riportano in primo piano la necessità di stadiare accuratamente i pazienti, per garantire loro il miglior trattamento possibile.
